全胃切除术患者经留置鼻空肠营养管与空肠造口行肠内营养的临床比较研究

摘　　要：目的比较全胃切除术患者经留置鼻空肠营养管与空肠造口行肠内营养的临床效果。方法将淮安市第一人民医院2006年7月至2010年12月进行全胃切除术治疗的57例患者按照肠内营养方法不同分为两组,留置鼻空肠营养管的30例作为对照组,空肠造口的27例作为观察组,比较分析两组的临床效果。结果观察组的肛门排气时间、排便时间及住院时间均明显短于对照组,且观察组术后第8d各项营养指标均明显优于对照组,组间差异均有统计学意义(P<0.05);观察组的并发症发生率为7.4%,略低于对照组的13.3%,但组间差异无统计学意义(P>0.05)。结论全胃切除术患者经空肠造口行肠内营养临床效果更为满意,能明显改善患者的营养状态,并且使患者胃肠功能恢复更快,有利于患者的康复,是较佳的肠内营养方法之一。Objective To compare the effect of enteral nutrition by indwelling nasogastric feeding tube and stonm of jejunum in patients with total gastreetomy. Methods From July 2006 to December 2010 in our hospital, 57 patients with total gastrectomy were randomly divided into two groups accord- ing to different methods of enteral nutrition, 30 patients with indwelling nasal jejunal feeding tube as the control group, 27 patients with stoma of jejunum as the observation group, the clinical results of two groups were compared. Results The anal exhaust time, defecation and hospital stay of observation group were significantly shorter than those in the control group, and the nutritional indices 8 days after operation of the observation group were significantly better than those in the control group, the differ- ences were statistically significant between two groups （P 〈 0. 05） ; The complication rate of observa- tion group was 7. 4%, slightly lower than 13.3% of the control group, but there was no significant difference between two groups （P 〉 0. 05 ）. Conclusions Total gastreetomy patients with jejunostomy enteral nutrition is more satisfactory clinical results, can significantly improve the nutritional status of patients, and the faster recovery of gastrointestinal function in patients, is conducive to the rehabilita- tion of patients, is a better way of enteral nutrition.
