摘  要:目的:探讨HIE新生儿心肌酶谱的变化及其与窒息后脑损伤的关系。方法:按病情程度将80例HIE分为轻度HIE组(30例)、中度HIE组(26例)和重度HIE组(24例);同期选择25例健康足月新生儿作对照(健康对照组)。采用全自动生化分析仪测定不同程度HIE患儿和健康对照组新生儿心肌酶谱[AST、CK、CK-MB、LDH、羟丁酸脱氢酶(HBDH)]活性及应用放射免疫法测定神经特异性烯醇化酶(NSE)水平。结果:①与健康对照组相比,HIE患儿血清心肌酶谱、NSE水平明显升高,差异有统计学意义(P<0.01);轻度HIE组、中度HIE组和重度HIE组患儿心肌酶谱、NSE水平随病情加重均逐渐升高,两组比较差异有统计学意义(P<0.01)。②HIE组患儿血清心肌酶和NSE水平均呈显著正相关(P<0.01)。③HIE预后差患儿心肌酶、NSE水平高于预后好者(P<0.01)。结论:心肌酶谱测定结果可作为早期诊断HIE心肌损伤的指标,并可作为判断HIE病情严重程度和预后的早期标志物。Objective:To explore the changes of myocardial enzymes in neonates with hypoxic-ischemic encephalopathy(HIE)and their relationships with brain injury after asphyxia.Methods:According to illness severity,80 cases with HIE were divided into mild HIE group(30 cases),moderate HIE group(26 cases),serious HIE group(24 cases),and 25 health neonates were regarded as healthy control group.The levels of myocardial enzymogram,neuron-specific enolase(NSE) in 80 different degree HIE newborns and 25 healthy newborns were measured by automatic biochemistry analyzer.Results:① Compared with healthy control group,the levels of myocardial enzymogram,NSE in HIE newborns were obviously increased,the differences were significant(P0.01),The levels of myocardial enzymogram and NSE were increased by turns in mild HIE,moderate HIE and serious HIE groups,the differences were significant(P0.01).② There were significant positive correlation relationship between myocardial enzymogram and NSE in HIE newborns(P0.01).③The levels of myocardial enzymogram and NSE in the bad prognosis group were higher than those of the good prognosis group(P0.01).Conclusion:The myocardial enzyme may be a early diagnostic index for myocardial injury with HIE,and they may be used as early markers for judging the severity and clinical prognosis of HIE.
