皮肌炎伴发肺间质病变的相关因素分析

摘　　要：目的了解皮肌炎肺间质病变的临床特点,探讨影响皮肌炎伴发肺间质病变(ILD)的危险因素。方法回顾性研究近3年来收治的83例皮肌炎(DM)患者临床资料,采用Logistic回归分析皮肌炎伴发ILD的影响因素。结果皮肌炎患者ILD的发生率为42.2%。筛选出皮肌炎伴发ILD的主要相关因素有Gottron's皮疹、恶性肿瘤、雷诺现象、干咳、关节痛和肌力,其中Gottron's皮疹及恶性肿瘤与ILD呈负相关,雷诺现象、干咳、关节痛和肌力与ILD呈正相关。结论Gottron's皮疹阳性或伴发恶性肿瘤的DM患者ILD的发生率降低,有雷诺现象、干咳、关节痛或肌力较好的DM患者发生ILD的概率较高。Objective To assess the factors related to the development of interstitial lung disease(ILD) in patients with dermatomyositis(DM).Methods The clinical data of 83 patients with dermatomyositis was collected and analyzed retrospectively.The factors related to the development of ILD were identif ied by Binary Logistic regression analysis.Results The prevalence rate of ILD in the total 83 patients with DM was 42.2%.The factors related to the development of ILD were Gottron's eruption,malignancy,Raynaud phenomenon,cough,arthralgia and muscle power grade.Among these factors,Gottron's eruption and malignancy were negatively correlated with the development of ILD.Raynaud phenomenon,cough,arthralgia and muscle power grade were positively correlated with the development of ILD.Conclusion The patients who presented with Gottron's eruption or malignancy are less likely to develop ILD,and those who showed Raynaud phenomenon,cough,arthralgia or higher muscle rank are more likely to develop ILD.
