煤工尘肺结核合并肺癌60例临床分析

摘　　要：目的探讨煤工尘肺结核合并肺癌的临床特点及其意义。方法对开滦(集团)有限责任公司唐家庄医院1990—2010年间收治1 452例煤工尘肺患者中合并结核的217例,合并肺癌的139例,对其中煤工尘肺结核又合并肺癌的60例进行回顾性分析。结果煤工尘肺合并肺结核发生率为14.9%,煤工尘肺合并肺癌发生率9.6%,60例煤工尘肺结核又合并肺癌发生率27.6%;煤工尘肺结核合并肺癌60例中尘肺Ⅰ期15例(25.0%),Ⅱ期26例(43.3%),Ⅲ期19例(31.7%);肺癌细胞学分类以鳞癌多见;煤工尘肺结核合并肺癌胸片病灶部位以双上肺叶居多,占56.7%;双下肺叶占31.7%;中叶占11.7%。结论煤工尘肺结核合并肺癌特点:①结核也是导致煤工尘肺患者高发肺癌的原因;②Ⅱ、Ⅲ期煤工尘肺结核合并肺癌高发;③肺癌细胞学分类以鳞癌多见;④胸部X线肺癌病灶多见于双上肺叶;⑤易误诊。[Objective]To explore the clinical characteristics and significance of Coal-worker＇s pneumonoconiosis（CWP） combined with lung cancer. [Methods]A total of 1 452 cases of CWP,including 217 combined with tuberculosis,139 cases combined with lung cancer were treated in Kailuan（Group） Co.,Ltd.Affiliated Tangjiazhuang Hospital from 1990-2010.Among them,60 cases combined with tuberculosis and lung cancer were retrospectively analyzed. [Results]The incidence of CWP combined with tuberculosis was14.9%,the incidence of CWP with lung cancer was 9.6%,that of CWP combined with tuberculosis and lung cancer was 27.6%.Of 60 cases,15（25.0%） were at CWP stage I,26（43.3%） with CWP stage II,19（31.7%） with stage III;most cases had squamous cell carcinomas by cytological classification of lung cancer.Most lesions located in double upper lobes,occupying 56.7%l,31.7% located in double lower lobes,11.7% in middle lobe. [Conclusion]The features of CWP combined with tuberculosis and lung cancer are ① tuberculosis also results in high incidence of lung cancer among CWP patients;②high incidence of tuberculosis with lung cancer among CWP patients at stage Ⅱand stage III;③most cases have squamous cell carcinomas according to cytological classification of lung cancer;④most lesions located in double upper lobes by chest X-ray;⑤It is diagnosed as other diseases easily.
