摘 要:目的 通过动物实验对人发角蛋白人工腱移植后肌腱粘连情况进行较为细致的研究。方法 以雄性成年鸡的趾深屈肌腱为实验模型,切除第二趾骨段相当于A2 -A4滑车间趾深屈肌腱0 . 5cm ,实验组用人发角蛋白人工腱(HHKAT)行肌腱移植,对照组用普通人发腱移植,分别于术后2、4、6、8周取材做大体病理观察,屈趾度及肌腱滑动度测量以及组织学观察,并进行比较。结果 实验组移植肌腱滑动功能及屈趾活动好,且粘连程度轻,与对照组相比差异有统计学意义(P <0. 0 5 ,P <0 . 0 1) ,组织学显示肌腱修复实验组优于对照组。结论 ①人发角蛋白人工腱移植后肌腱粘连程度较普通人发腱轻微。②人发角蛋白人工腱移植后修复主要依靠“外源性愈合方式”,肌腱滑动功能主要依赖术后早期的功能活动。Objective To analyse the tendon adhesion aftter transplantation of human-hair keratin artificial tendon thriugh animal experiment. Methods Flexor digitorum profundus of adult male hens was used in the experiment.The long flexor tendons between A2 and A4 were excised,experimental group received tendon graft with HHKAT and control group with general human-haor tendon.Specimens were removed at different intervals post operatively and were studied with:Gross and histologic observations;Degree of toe flexion and tendon gliding;Grads of tendon adhesion. Results The function recovery of the active and passive flexion and the tendon gliding were significantly better in the experimental group( P <0.05, P <0.01);The extent of tendon adhesion was less in the experimental group( P <0.05);Histological study demonstrated that tendonhealing in experimental group was better than control group. Conclusions ①After tendon transplantation,the extent of tendon adhesion of HHKAT is less than that of general human-hair tendon.②Healing of the HHKATTS is mainly by an extrinsic mechanism.The function recovery of tendon gliding mainly depended upon early exercise after tendon grafting.
