手术治疗下肢深静脉血栓后遗症的临床研究

摘　　要：目的 探讨下肢深静脉血栓后遗症的诊断方法和外科手术治疗的临床价值。方法 回顾分析1992年以来手术治疗下肢深静脉血栓后遗症38例42条下肢的临床资料。所有患者均经血管造影确诊,准确率为100％,Ⅰ型6条;ⅡA型5条,ⅡB型14条,Ⅲ型17条。其中全肢型8条,节段型25条,混合型9条。人造血管股浅静脉环缩术18条;股浅静脉瓣膜修复术4条;大隐静脉-腘静脉原位转流术6条;腘静脉外肌袢成形术4条;髂总静脉-股浅静脉人造血管移植转流术6条;大隐静脉-股浅静脉交叉转流术4条。结果 治疗后症状完全消失32条(76.2％),部分消失或改善10条。结论 血管造影是诊断下肢深静脉血栓后遗症的金指标,可根据瓣膜破坏及通畅程度,选择不同的手术方法治疗。Objective To study the effective surgical methods for the sequela of deep vein thrombosis (DVT). Methods The clinical data of 38 patients with the sequela of DVT 42 limbs,treated by surgery since 1992 were analysed retrospectively,diagnosis accuracy rate of angiography sequela of deep vein thrombosis is 100%. In this series,there were type Ⅰ in 6 limbs;type Ⅱ A in 5;type Ⅱ B in 14;type Ⅱ in 17. Among all limb type 8 limbs,modosityp segmentum type 25 limbs,mixed type 9 limbs. The artificial blood vessel cohort vein ring operative 18 limbs; cohort vein valvula prothetics 4 limbs; meg-saphena and popliteal vein insitu trans-current 6 limbs;popliteal vein outer muscle ansa shaping 4 limbs;vena iliaca communis and cohort vein artificial blood -vessel tansplantion trans-current 6 limbs; meg-saphena cohort vein intercross trans-current 4 limbs. Results The after treatment of symptom total die out 32 limbs(76.2%).partly die out 10 limbs. Conclusion The angiography vasography is diagnosis chrys-indicatrix of the deep vein thrombosis sequela. In the light of valve undoing degree, preference unequal surgical methods treatment.
