摘　　要：目的 评价在麻风高流行县（市）开展消除麻风运动，结合常规病例发现工作发现麻风病例的效果。方法 广泛开展社区的麻风健康教育，培训基层医务人员的麻风防治技能，开展麻风线索调查和接触者检查，确诊和治疗患者。结果 6个麻风高流行县（市） 1999年发现 122例新发病例，发现率为 3.7/10万，比过去 3年年均发现病例数（ 67例）和发现率（ 2/10万）高近 1倍，其中家庭内接触者发病占 47％。结论 消除麻风运动结合常规病例发现工作可促进社区“积压病例”的发现。在麻风流行较严重地区，尤其在远离县城和难以到达的村庄，在进行充分健康教育和社区培训的基础上，采用主动和被动相结合，通过皮肤科门诊、接触者检查和线索调查可发现绝大多数病例。Objective To evaluate the effectiveness of Leprosy Elimination Compaigns (LEC) integrated with routine case- finding of leprosy in the high leprosy- epidemic areas in Yunnan, Guizhou and Sichuan Provinces. Methods Extensive health education of leprosy in the community, training of paramedical workers on leprosy, clue survey and contact survey were carried out to make diagnosis and provide treatment. Results A total of 122 new leprosy cases were detected in 6 high leprosy- epidemic counties through LEC in 1999 with a detection rate of 3.7/100 000, but in contrast, the annual number of newly detected leprosy cases was 67 cases with an average detection rate of 2/100 000 during the period of 3 years before LEC. Among the newly detected leprosy cases, about 47％ were leprosy household contacts. Conclusion LEC combined with routine activities of case finding can improve the detection of the back- log cases in the communities. In the high epidemic areas, especially in the remote and difficult- to- access areas, majority of leprosy cases can be detected through combining active and passive case- finding methods, providing extensive health education, improving leprosy services in skin clinic, leprosy contact and clue surveys.
