摘  要:目的 :探讨经皮冠状动脉腔内成形术 (PTCA)和冠脉内支架术对不稳定型心绞痛的胸痛缓解、心绞痛复发、急性心肌梗塞的发生等影响。方法 :47例临床诊断为不稳定型心绞痛 ,除外急性心肌梗塞。入院后经正规内科治疗 4~ 7d后接受PTCA和冠脉内支架术治疗。术后继续药物治疗 ,并防再狭窄治疗。结果 :47例中的 42例药物治疗有效 ,5例疗效不满意 ,44例成功地实施了PTCA和支架档 ,成功率 93.6 % ,3例因导引导丝无法通过狭窄或完全闭塞处而失败 ,44例的 78支冠状动脉进行了PTCA ,其中 73支血管置入了 87枚支架 ,5支因扩张效果满意未置入支架 ,支架植入成功率 10 0 %。所有患者心绞痛均得到明显缓解 ,无一例急性心肌梗塞和死亡发生。随访 2~ 2 6个月 ,3例复发心绞痛 ,1例急性心肌梗塞 ,无死亡。结论 :PTCA和支架术治疗不稳定型心绞痛具有缓解心绞痛彻底 ,复发率低 ,改善患者预后 ,安全 。Objective:To investigate the effects of percutaneous tranluninal coronary angioplasty (PTCA) and stenting on the symptom alleviation, recurrence of angina pectoris,and attack of Acute Myocardial infarction(AMI) in patients with unstable angina pectoris.Material and Methods: 47 patiants with UA were diagnosed and excluded from AMI.All patients received PTCA and stenting after being treated for 4 to 7 days with a stardard physical method.All patients was continually treated with pharmacotherapy and prevented from instenting stenosis.Results: 42 patients responded well to the medicine,while 5 patients did not.44 patients successfully received PTCA and were trimphfully placed stents.3 patients did not responded at all to PTCA because wires did not pass through stenosis or stopup.There were 78 coronary arteries placed with 87 stents.The successire percent of planting stents was 100%.All patients gained obvious mitigation.No one had an attack of AMI or sudden death.Affter their tracks for 2 to 26 months,3 patients had an attack of UA,1 patient had AMI,and no one dead.conclusion:Treating uA with PTCA and stentings has some advantages such as obvious mitigation,lower recurrent rate,safe,and improvement of prognosis.
