摘 要:目的 :设计带压力监测装置的呼吸复苏气囊 ,为在紧张的心肺复苏中帮助急诊医师随时掌握病人近端气道压力的大小。方法 :心搏骤停或呼衰病人气管插管时做血气分析 ,用带压力监测装置的复苏气囊面罩正压通气或插管后用该气囊正压通气 30min ,压力控制在黄色区 (4kPa)以内 ,然后连接人工呼吸机辅助通气 ,复查血气分析及胸部X线摄片。结果 :使用该装置前后之血气分析比较 ,动脉氧分压升高、动脉二氧化碳分压降低、血氧饱和度升高 (P <0 .0 1)。胸部X线摄片未发现气胸。结论 :本研究装置体积小、重量轻、操作简便、使用安全 ,值得在各级急救站、医院急诊科。Objective:The lung compliances of patients are different depending on different underlying diseases.In order to help emergency doctors to control proximalairway pressure,we have designed the resuscitator bag with pressure monitoring.Methods:blood gas analysis of patients with cardiac arrest or respitratory failure was performed before endotracheal intubation,ventilatedpatients through the mask or endotracheal tube for 30 minutes.With pressure of less than 4 kPa,followed by connecting patients to ventilator.A chest Xray examination and blood gas analysis were token.Results:After using the resuscitator bag,PaO 2 and SaO 2 were increased and PaCO 2 was decreased,( P <0.01).No peneumothorax was found.Conclusion:At present,none of resuscitator in the world has pressure-monitoring design for safety.It likely causes barotrauma by unskilled operator during CPR.This device is of small size,light,easily using,and safety.It can be used widely in emergency and ICU.
