HBsAg阴性/抗HBc阳性献血员HBV感染状况调查

摘　　要：目的 对431例HBsAg阴性献血员检测抗-HBc、抗-HBs,抗-HBc阳性者196例(45.5％),其中单项抗-HBc阳性者35例(17.9％),抗-HBc/抗-HBs阳性者161例(82.1％)。对抗-HBc阳性者检测抗-HBc IgM和HBV DNA(聚合酶链法),抗-HBc IgM的检出率为32.1％(63/196),其中抗-HBc/抗-HBs阳性者检出率为29.8％(48/161),单项抗-HBc阳性者检出率为42.9％(13/35),二者无差异;HBV DNA检出率为14.8％(29/196),单项抗-HBc阳性者HBV DNA检出率为25.7％,显著高于抗-HBs/抗-HBc阳性者(12.4％)(P<0.05);抗-HBc IgM阳性者HBV DNA检出率(39.7％)也显著高于抗-HBc IgM阴性者(3.0％)(P<0.001),二者阴阳性符合率则为78.6％(154/196)。HBsAg阴性/抗-HBc阳性献血员仍有传染性存在,尤以抗-HBc IgM阳性者最具血源传播HBV的危险性,因此,建议对HBsAg阴性献血员再进一步筛检抗-HBc IgM。Objective Sera from 431 HBsAg-negative blood donors were examined for anti-HBc and anti-HBs,and anti-HBc positive donors were furhter tested for IgM anti-HBc by ELISA and HBV DNA by polymerase chain reaction(PCR). Among 431 donors, 196(45. 5%) donors were positive for anti-HBc. Of them, 35( 17. 9%) donors were positive for anti-HBc only and 161(82.1 % ) for both anti-HBc and anti-HBs. Positive rate of IgM anti-HBc in the anti-HBc-positive donors was 32.1 % (63/196), and that in donors with anti-HBc/anti-HBs positive and anti-HBc positive only were 29. 8% (48/161) and 42.95 (15/35) (P > 0.1), respectively. Positive rate of HBV DNA was 14.85(29/196) in donors with anti-HBc,and that in donors with anti-HBc positive only and anti-HBC/an-ti-HBs positive were 25. 75(9/35) and 12.4% (20/161) (P < 0.05) .respectively. Positive rates of HBV DNA in donors with IgM anti-HBc positive and negative were 39. 7% (25/63) and 3.0% (4/133) (P < 0.001) respectively. 154(78.6% ) of donors with anti-HBc were negative or positive only for both IgM anti-HBc and HBV DNA. It is suggested that HBV infection may exist in blood donors with HBsAg negative and anti-HBc positive, especially in donors with IgM anti-HBc positive. The transfusion risk of hepatitis B virus infection could be reduced further if we screen for IgM anti-HBc for better in HBsAg-negative donor.
