经鼓室进路施行膝状神经节和面神经减压术

摘　　要：目的　探讨定位诊断为膝状神经节病变的面瘫，经鼓室进路行面神经减压。　方法　对５ 例分别为颞骨骨折、贝尔氏面瘫、颞骨先天性胆脂瘤病例，经鼓室进路循面神经水平段，在鼓室前隐窝内下骨板磨开齿突上方面神经骨管，暴露面神经迷路段，行面神经膝状神经节减压。　结果　５ 例面瘫均获得满意恢复，３ 例恢复了部分听力。　结论　对膝状神经节的外伤和病变，利用本术式，只要操作细致，不损伤手术区附近的水平半规管和前庭窗。Decompression　operation　of　geniculate　ganglion　by　the　tympanic　approach$$$$ QIN　Xueling,　WEN　Dinhou,　TANG　Baocheng Department　of　Otolaryngology,The　First　Affiliated　Hospital,West　China　University　of　Medical　Sciences,Chengdu　610041 【Abstract】 Objective This　study　for　facial　paralysis　with　the　sit　of　lesion　test　was　decompressed　operation　by　the　tympanic　approach Method Five　patients　suffering　the　temporal　bone　fractures,Bells　palsy　and　congenital　cholesteatoma　in　temporal　bone,were　managed　to　open　the　facial　nerve　bone　tube　by　tympanic　and　tympanic　portion　of　facial　nerve　approach　and　exposed　the　labyrinthitis　portion　of　facial　nerve　between　incus　and　plate　of　middle　cranial　fossa　above　the　processus　cochleariformis Result Postoperatively　all　case　has got　marked　improvement, and hearing　function　of　3　cases　be recovered Conclusion If　the　decompressive　operation　of　facial　nerve　were　carefully　carried　and　the　horizontal　tube　and　oval　window　not　be　damaged,the　facial　nerve　function　and　hearing　function　can　be　able　to　improve
