肥厚型心肌病的特殊表现形式

摘　　要：目的分析不典型肥厚型心肌病的特殊表现（临床及影像），提高临床诊断能力。方法系统分析9例病人的病史、临床表现及其影像学特征，互为补充印证以得出正确的结论。结果5例以胸痛伴缺血性ST－T改变者中3例被诊断为左室非对称肥厚型，2例确诊为心失肥厚型；2例以心源性哮喘为特征者均被诊断为左室梗阻型；2例以进行性浮肿伴浆膜腔积液为特征者1例确诊为双心室肥厚型，另1例则为肥厚型心肌病伴进行性心脏扩大。结论重视肥厚型心肌病的特殊表现，综合系统分析是得出正确诊断的关键。Objectives Analyzing the special clinical and imaging manifestation of theatypical hypertrophic cardiomyopathy (HCM) to improve the ability of diagnosis and differentialdiagnosis. Methods To analyze 9 patients' histiory, clinical and imaging features systmaticallyfor finally acquiring correcctive results from comparison of them each other. Results Out of 5patients0 with chest pain and ischemic ST -T wave changes (electrocardiogram), 3 cases were ofasymmnetry hypertrothy and 2 cases apical hrpemepyh. Two patients with cardiac asthma were di-agnosed as obstructive HCM. One patients was diagnosed as dual ventricular hypertrophic car-diomyopathy and another as HCM with progressive left ventricular dilatation in 2 cases of progres-sive edema and dropsy of serous cavity. Conclusion: It is a key to acquire corrective diagnosisthrough comprehensive analysis of the special features of HCM patients.[
