心肌肌钙蛋白T与急性心肌梗死患者左心室功能的关系

摘　　要：目的探讨急性心肌梗死（AMI）后血清心肌肌钙蛋白T（cTnT）峰值与心功能、左室射血分数（LVEF）的关系。方法分别测定了24例急性前壁心肌梗死患者的cTnT、心功能、LVEF、EDV、ESV。结果所有前壁AMI患者的cTnT峰值与心功能成等级相关，与LVEF成负相关，与EDV、ESV成正相关；随访6个月，3例死于心衰的病人cTnT峰值均＞15μg／L（平均为17．2±1．9）μg／L），LVEF值下降者的cTnT峰值在8～15μg／L之间，LVEF值升高者的cTnT峰值均＜μg／L。结论前壁AMI后cTnT峰值越高，心功能越差，预后也越差。Objective To evaluate the relationship between the peak of cardiac troponin T (cTnT ) and cardiac function ,i.e.left ventricle ejection fraction (LVEF) in patients with acute myocardiac farction (AMI). Methods The serum levels of cTnT was tested and LVEF, left ventricular end- diastolic volume(EDV) and end- systolic volume(ESV) were detendned by Simpson biped formula in M ents with forewatI AMI. ROots aampeaks Of cTnT in 24 cases were condated tO cedacfunehon, EDV and ESV, and neghvely cormlated toLVEF. Dining 6 baths follow - up, the Peak valueof cTnT in 3 died patients was > 15 afL(mam(17.2 1.9) afL) and in those with decreasedLVEF was 8 15 ig/L, but in those with increasedLVEF was < 8 afL. Conelusion acer forewallAMI, the higher the Peak value of cTnT, the poolerthe left ventricular function and prois.[
