经尿道RevoLix 2μm激光前列腺切除术对患者性功能的影响

摘　　要：目的:探讨采用RevoLix 2μm激光手术系统治疗BPH对患者性功能的影响。方法:对120例BPH患者进行了RevoLix 2μm激光手术治疗,全部病例均行IPSS评分。利用国际勃起功能障碍问卷-5(IIEF-5)和自制统一的射精障碍调查表,分别于术前、术后半年了解患者阴茎勃起功能情况、性欲、射精情况及对性生活满意程度进行调查。结果:全部手术均成功,IPSS有明显改善,从(25.12±3.21)分下降为(6.26±2.12)分,(P<0.05)。术后患者均保留性功能。IIEF-5术前评分为(23.50±3.12)分,术后半年为(22.40±4.62)分;术前有性欲者占81.67%,术后半年占85.00%,手术前后比较差异无统计学意义(P>0.05)。术前射精异常者占23.33%,术后半年射精异常者占71.67%,术后射精功能较术前明显下降(P<0.05)。术前对性生活满意者占65.00%,术后占70.00%,手术前后比较差异无统计学意义(P>0.05)。结论:RevoLix 2μm激光手术系统治疗BPH有效安全,术中、术后并发症少,对患者阴茎勃起功能、对性生活满意程度及性欲无明显影响,其影响主要为射精障碍。术中操作仔细,防止损伤影响阴茎勃起的神经血管束,术后处理得当,可减少或避免对患者的性功能影响。Objective：To evaluate the impact of transurethral RevoLix 2 micron continuous wave laser vaporesection of prostate on sexual functions of the patients with benign prostatic hyperplasia（BPH）. Methods： A total of 120 aged patients with BPH who reported normal sexual relation before operation were followed up for 6 months in this study, the International Prostate Symptom Score （IPSS）,the International erectile dysfunction questionnaire （fIEF-5） and a self-designed ejaculatory questionnaire were used before operation and 6 months later after opera tion. The incidence of erectile dysfunction, index of sexual activation including sexual interest, pleasure in sex, sexual climax, satisfaction for sexual intercourse were recorded. Results：The RevoLix 2 micron continuous wave laser vaporesection was successfully performed on all the 120 patients with BPH. The IPSS improved from （25.12±3.21） to （6.26±2.12）（P〈0.05）after operation, the questionnaire result of postoperative IlEF-5 was not sig nificantly decreased than preoperative score,（23.50±3.12 vs. 22.40±4.62,P〉0.05）. The satisfactory with sex life was not changed significantly before and after operation （65.00% vs. 70.00% ,P〉0.05）. The sexual desire also was not changed after operation, but ejaculatory dysfunction increased from 23.33 % to 71.67% （P（0.05）. ）Conclusions： RevoLix 2 micron continuous wave laser vaporesection of prostate is an excellent treatment modality for BPH, it shows no statistical differences in the incidence of ED, satisfactory with sex life and sexual desire, but the ejaculatory dysfunction increases signifieantly.
