摘  要:目的观察盲肠结扎穿孔(CLP)大鼠MHC-Ⅱ+/CD14+的变化趋势及其与动物死亡的关系,探讨MHC-Ⅱ+/CD14+对脓毒症大鼠免疫功能监测及预后预测的价值。方法采用大鼠经典CLP脓毒症模型,40只实验动物随机均分为正常对照组(C组)、假手术组(S组)、盲肠穿孔1孔组(P1组)和盲肠穿孔2孔组(P2组)。S、P1、P2组动物在制模成功后1、2、3、4d抽血,C组仅在第4d抽血,流式细胞仪检测MHC-Ⅱ+/CD14+水平,并记录每天各组动物死亡只数。同时根据MHC-Ⅱ+/CD14+水平分为≥40%组、30%~40%组、<30%组,记录各组动物死亡数目。结果采用Chiss软件进行统计学分析。结果与C组、S组比较,P1组在制模成功后第2、3、4天,P2组在制模成功后第1、2、3、4天累计死亡数均明显增加(P<0.05),而C组与S组间、P1组与P2组间比较差异无统计学意义(P>0.05)。与C组、S组比较,P1组、P2组CLP小鼠MHC-Ⅱ+/CD14+水平在制模成功后第1、2、3、4天均明显下降(P<0.05或P<0.01),第2天为最低值,且P2组较P1组下降更明显(P<0.01),但C组与S组间比较差异无统计学意义。MHC-Ⅱ+/CD14+水平<30%组死亡数明显高于≥40%组(P<0.05)及30%~40%组(P<0.05),而≥40%组与30%~40%组间比较差异无统计学意义。结论 MHC-Ⅱ+/CD14+水平可用于脓毒症大鼠免疫功能状态的监测及预后预测。Objective To observe the changing trend of MHC-Ⅱ+/CD14+ in rats that underwent CLP(Cecal Ligation and Puncture) and its relationship with mortality and examine the significance of MHC-Ⅱ+/CD14+ in monitoring immunologic function and prognostication of septic rats.Methods Classic CLP sepsis models were established among rats.A total of 40 rats were randomly distributed into four groups,the normal control group(C),the sham operation group(S),the one cecal perforation group(P1),and the two cecal perforations group(P2).Blood samples were taken to test the levels of MHC-Ⅱ+/CD14+ by flow cytometer on days 1,2,3,and 4 for groups S,P1,and P2,and on day 4 for group C.The mortality was recorded every day.(2).Based on the levels of MHC-Ⅱ+/CD14+,the rats were divided into three groups(≥40%,30%-40%,and 〈30%).The mortality for each group was recorded.The data were processed using Chiss software.The data are expressed as ±s and the MHC-Ⅱ+/CD14+ levels in each group was analyzed with an F test.The mortality rate in each group and its relationship with the level of MHC-Ⅱ+/CD14+ were analyzed using the exact probability method(P〈0.05,with significant differences).Results Compared with groups C and S,group P1 had apparently higher mortalities on day 2,day 3 and day 4 after successful modeling and P2 does so at days 1,2,3,and 4(P〈0.05).The differences between groups C and S and between groups P1 and P2 had no statistical significance(P〉0.05).Compared with groups C and S(days 1,2,3,and 4 after successful modeling),the CLP rats in groups P1 and P2 had apparently reduced levels of MHC-Ⅱ+/CD14+ at all time points observed(P〈0.05 and P〈0.01),the lowest on day 2.Group P2 drops more significantly than group P1(P〈0.01),but the difference between groups C and S had no statistical difference.The group with MHC-Ⅱ+/CD14+ 〈30% had more mortalities than groups ≥40%(P〈0.05) and 30%-40%(P〈0.05).The difference in mortal
