摘  要:目的 :研究系统性高血压在糖尿病肾病发病机理中的作用 ,以及血管紧张素转换酶抑制剂开搏通 ( Captopril)治疗自发性高血压 ( SHR)糖尿病 ( DM)大鼠对肾功能的影响。  方法 :建立起自发性高血压大鼠链脲菌素 ( STZ)诱导的糖尿病动物模型 ( SHR+DM) ,设立开搏通治疗组、常规三联降压组 (利血平 +肼苯哒嗪 +双氢克尿噻 )、未治疗组及正常对照组。测定 4周及 16周时肾功能包括尿白蛋白排泄量、肌酐清除率 ( Ccr)以及肾小球面积。  结果 :与正常血压 WKY大鼠比较 ,4周及 16周的早期糖尿病 SHR大鼠即已存在肾功能变化 ,提示高血压可能在糖尿病肾病的发展中起重要作用。  结论 :开搏通及常规三联降压治疗既能使 SHR+DM大鼠的血压降至正常 ,又可使肾功能变化得到改善 。Objectives:To study the role of systemic hypertension on the progression of diabetic nephropathy and to evaluate the effects of anti - hypertensive therapy on renal in- juries in SHR with diabetes.Methods:Albumin excretion rate,creatinine clearance and glomerular tuft areas were determined on the 4th and the 1 6 th weeks in streptozotocin (STZ) treated spontaneously hypertensive rat(SHR) .The rats were randomly divided into five groups,that is wistar kyoto rat (WKY) (n =1 6 ) for normotensive control,SHR (n= 1 6 ) for hypertensive control,SHR diabetic rat(SHR+ DM) (n=1 6 ) ,SHR+ DM with capot- pril(SHR+ DM+ C,n=1 6 ) ,and SHR+ DM with triple antihypetensive therapy(reser- pine,hydrolazine,hydrochrothiazide) (SHR+ DM+ T,n=1 6 ) . Results:In SHR+ DM,uri- nary albumin excretion,creatinine clearance and glomerular tuft areas increased significantly with age,glomerular tuftarea and urinary albumin excretion markedly increased as compared with that in WKY.Those abnormalities in SHR diabetic rats were significantly improved by antihypertensive therapy,either captopril or routine triple antihypertensive therapy.It sug- gests that both systemic and glomerular hypertension may contribute to the development of diabetic nephropathy.Conclusions:Antihypertensive therapy,especially captopril(angiotesin converting- enzyme inhibitor) ,is an important intervention against diabetic nephropathy in SHR with diabetes.
