胶体次枸橼酸铋合并疗法根除幽门螺杆菌的多中心研究

摘　　要：本文用胶体次枸橼酸铋（ＣＢＳ）合并呋喃唑酮二联疗法和ＣＢＳ、呋喃唑酮合并甲硝唑三联疗法治疗幽门螺杆菌（ＨＰ）阳性的慢性胃炎７４例、胃溃疡４０例和十二指肠溃疡７１例，旨在探讨ＨＰ的根除率和消化性溃疡的愈合率。结果表明，二联疗法２周和三联疗法１０天的ＨＰ根除率分别为７３．３％（６６／９０）和７７．８％（７４／９５）；８周中，二联疗法的胃、十二指肠溃疡愈合有效率分别为８８．８％（１６／１８）和８５．７％（３０／３５），三联疗法的胃、十二指肠溃疡愈合有效率分别为９０．９％（２０／２２）和９１．６％（３３／３６）。二组间的ＨＰ根除率以及胃、十二指肠溃疡愈合有效率均无显著性差别（Ｐ＞０．０５）。消化性溃疡治疗后有效者的ＨＰ大多得到根除，治疗后无效者，ＨＰ多仍阳性，两者有显著性差异（Ｐ＞０．０１）．三联疗法的副反应发生率显著高于二联疗法（Ｐ＞０．０１）。CBS and Furazolidone have been proved to be effective for the eradication of H. pylori. Our objective was to evaluate the dual therapy(the combination of CBS 120 mg q.i.d. and Furazolidone 100 mg q.i.d.for 14 days)and triple therapy(the combination of CBS 120 mg q.i.d.,Furazolidone 100 mg q.i.d.and Metronidazole 200 mg q.i.d.for 10 days)In the treatment of H.pylori infecton.185 patients with H.pylori associated gastroduodenal diseases were randomly allocated into dual therapy and triple therapy groups.Among them,74 patients were non-ulcer dyspepsia,71 duodenal ulcer (DU) and 40 gastric ulcer(GU). The results showed that in dual therapy group the eradication rate of H.pylori was 73.3% and the healing rate of DU and GU were 80% and 77.7%,respectively.In triple therapy group the eradication rate of H.pylori was 77.8% and the healing rates of DU and GU were 86.1%and 77.3%,respectively.There were no significant differences in the eradication rate of H. pylori and the healing rates of DU and GU between dual therapy and triple therapy.There were more mild side-effects occurred in patients receiving triple therapy(P<0.01).It Is concluded that both the dual therapy and triple therapy are effective in the treatment of H.pylori associated gastroduodenal diseases,and are well tolerated in most patients.
