胶囊微量法^（14）C—尿素呼气试验检测幽门螺杆菌感染的初步研究

摘　　要：本文建立了胶囊微量法１４Ｃ－尿素呼气试验（１４Ｃ－ＵＢＴ）用于检测幽门螺杆菌感染。应用１μＣｉ（３７ＫＢｑ）１４Ｃ－尿素，胶囊给药。在收集５，１０，１５，２０，２５，３０，４５，６０分钟呼气检测的基础上建立了以１４ＣＯ２呼出高峰期（２５分钟），直接以ｄｐｍ／ｍｍｏｌＣＯ２表示的检测方法，并以Ｈｐ细菌学培养及病理组织学作为金标准对比。共检测３９例病人，Ｈｐ阳性２０例，Ｈｐ阴性１９例，结果显示胶囊微量法１４Ｃ－ＵＢＴ的敏感性为９３．３％，特异性９２．３％，阳性预检值９３．３％，阴性预检值９２．３％，诊断符合率达９２．９％。本组资料证明胶囊微量法具有安全、快速、准确、简便、经济等优点，可以代替传统的５－１０μＣｉ，大剂量１４Ｃ－尿素呼气试验。A microdose capsule-based 14C-urea breath test(14C-UBT)was developed to diagnose Helicobacter pylon(HP)infecton in 39 patients.Breath samples were collected at 5,10,15,20,25,30,45,60 min.The peak amount of 14C exhaled was at 25min.The results off14C-UBT were compared with those of Hp culture and histologic examination in antral biopsy obtained endoscopically in 39 patients.Using the mean value+3 SD as the cut off value,this test has the sensitivity of 93.3%,specificity of 92.3%,positive predictive value of 93.3% and negative predictive value of 92.3%.Our data shows that microdose capsule-based 14C-UBT is safe,rapid,simple,accurate and economical.
