冠心病心脏功能解剖的超声研究──附16例冠状动脉造影的对照分析

摘　　要：本文对16例经选择性冠状动脉造影（SCA）的陈旧性心肌梗塞（OMI）患者应用多普勒超声心动图（UCG）进行室壁运动评分（WMS），与SCA结果对照，发现冠状动脉病变支数愈多，出现节段性室壁运动异常（RWMA）的节段愈多，WMS亦愈高；冠状动脉狭窄程度≥75％的17支中，有16支在其供血的相应部位出现了RWMA，敏感性为94．1％。对该16例OMI患者测定心功能，发现左心舒张功能（LVDF）明显减退，而右心舒张功能（RVDF）无同步改变。结果表明：UCG对OMI定位诊断较敏感，通过观察RWMA及其WMS可推测冠状动脉受累程度，结合心功能测算有助于评估患者的心功能及预后。文中对RWMA与冠状动脉分支供血部位的对应关系作了探讨。Doppler echocardiography was used to investigate 16 patients with old myocardial infarction(OMI)subjected to selective coronary angiography(SCA).The left ventricular wall motion was observed and wall motion score(WMS)criterion was used.When compared with the result of SCA,it was found that the more the diseased branches of coronary，the more severe the segments of repional wall motion abnormality was(RWMA),and the higher the WMS.Of 17 branches with over 75％ stenosis observed by angiography,16 occurred RWMA in the corresponding blood-supplied locations.The sensitivity of localization was 94.1％.In 16 patients with OMI，the left ventricular diastolic function(LVDF)decreased significantly，but the right ventricular diastolic function(RVDF)had no change．The result showed that：the sensitivity of localization of UCG was good．The degree of impairment of coronary artery may be established by obsesrving RWMA and its score,in combination with the measurement of cardiae function indices．The heart function and prognosis of patients can be estimated．The corresponding relationship between RWMA and the blood-supplied localization of the coronary branches was discussed in this article．
