瑞舒伐他汀钙联合缬沙坦治疗早期糖尿病肾病疗效观察

摘　　要：目的探讨瑞舒伐他汀钙联合缬沙坦对早期糖尿病肾病(DN)的保护作用。方法将97例早期DN患者随机分为对照组、缬沙坦单药治疗组(单药治疗组)、瑞舒伐他汀钙与缬沙坦联合治疗组(联合治疗组)。3组患者均行常规降糖治疗,单药治疗组在常规降糖治疗基础上加用缬沙坦80 mg/d,联合治疗组在常规降糖治疗基础上加用缬沙坦80 mg/d和瑞舒伐他汀钙片10 mg/d。疗程均为12周。检测并比较治疗前后3组患者血糖[空腹血糖(FBG)、餐后血糖(PBG)]、血脂[总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、糖化血红蛋白(HbA1c)、血肌酐(SCr)及24 h尿微量清蛋白排泄率(UAER)等指标的变化。结果治疗后,联合治疗组患者血脂[TC、TG、LDL-C]、血糖(FBG、PBG)、HbA1c、UAER均较治疗前显著降低,差异有统计学意义(P<0.05);且治疗后联合治疗组与单药治疗组比较,血脂、UAER也显著降低,差异有统计学意义(P<0.05);3组患者SCr水平无论在治疗前还是治疗后,差异均无统计学意义(P>0.05)。结论瑞舒伐他汀钙与缬沙坦联合治疗早期DN,不仅能降低血脂和血压,还能减少患者的UAER,可有效预防和延缓早期糖尿病肾损害的进展。Objective To observe the clinical efficacy of Rosuvastatin combined with Valsartan in early diabetic nephropathy.Methods A total of 97 patients of early diabetic nephropathy were randomly divided into 3 groups（the Valsartan therapy group,the Rosuvastatin and Valsartan combined group and the control group）.All the three groups received routine treatment,the Valsartan therapy group received additional oral valsartan（80 mg/d）,and the combined group received additional oral Rosuvastatin（10 mg/d） combined with valsartan（80 mg/d）,all for 12 weeks.Blood glucose,HbA1c,serum lipids（TC,TG,HDL-C and LDL-C）,SCr,and 24 h urinary albumin excretion（UAER） were measured and compared among the three groups before and after 12 weeks of treatment.Results After treatment,blood glucose（fasting glucose and postprandial glucose）,HbA1c,serum lipids（TC,TG and LDL-C）,and UAER were significantly reduced in the combined therapy group than in the other two groups（P0.05）.Compared with the Valsartan group,blood lipids and UAER in the combined group were significantly lower（P0.05）.SCr levels were not significantly different before and after treatment in all groups（P0.05）.Conclusion Rosuvastatin combined with valsartan decreases the levels of blood lipids and blood pressure,as well as UAER.This therapy effectively prevents renal damages and delays the progress of early diabetic nephropathy.
