摘 要:目的为评价母传乙肝抗体水平不同的新生儿接种乙肝疫苗后的免疫学效果。方法采用随机入选自身对照研究,筛选出HBsAg和HBeAg双阴性孕妇和新生儿作为观察对象,分别采脐带血和常规乙肝疫苗三针接种后一个月的血,观察母传乙肝抗体水平及对乙肝疫苗免疫成功率的影响。结果婴儿母传乙肝抗体含量随着孕妇体内乙肝抗体含量的增高而增高,最高可传递给婴儿达1 000 mIU/ml。常规乙肝疫苗三针免后一个月的免疫成功率,由免前脐带血保护率42.3%上升至97.6%,抗体几何平均滴度由免前脐带血GMT 1:12.1上升到免后GMT 1:290.8。结论母体乙肝抗体可通过胎盘传递给新生儿,且新生儿乙肝疫苗接种效果不受母传乙肝抗体水平的干扰,并有免疫增强作用。Objective To evaluate the immunity effects of hepatitis B vaccine for infants with the different levels of maternal transferred antibody. Methods A randomized self-control study was employed to select HBsAg and HBeAg double-negative pregnant women and infants to serve as the objects of observation.The blood samples of the umbilical cord and the blood samples of infants after routine vaccination were collected to observe the levels of maternal transferred antibodies and the immunity effects of hepatitis B vaccine for infants Results The contents of maternal transferred antibodies were followed with the antibody levels in pregnant women,which was up to 1,000 mIU/ml of antibody titer on the transmission to infants by the pregnant women.42.3% of protection rates and GMT 1:12.1 before inoculation increased to 97.6% of protection rates and GMT 1:290.8 after inoculation on the immunity effects of hepatitis B vaccine for infants. Conclusions Hepatitis B antibodies of the pregnant woman were transmitted to the newborn by the placenta.The immunity effects of hepatitis B vaccine for infants were not interfered to the maternal transferred antibody.
