不同浓度罗哌卡因复合舒芬太尼用于剖宫产术后镇痛的临床研究

摘　　要：[目的]观察不同浓度的罗哌卡因复合舒芬太尼用于子宫下段剖宫产术后镇痛的临床效果。[方法]选取我院2009年10月至2010年10月120例拟行子宫下段剖宫产术后硬膜外镇痛的初产妇,随机分为A、B、C、D 4组,每组30例,分别予以0.075%、0.1%、0.125%、0.15%罗哌卡因＋0.2μg.mL-1舒芬太尼进行硬膜外镇痛,记录各组产妇术后2、4、8、12、24、48 h的镇痛VAS评分、运动阻滞评分以及不良反应等指标,组间进行比较。[结果]A组镇痛效果欠佳,术后2、4、8、12、24、48hVAS评分与其他3个组比较差异有统计学意义（P〈0.05）,B、C、D 3组间差异比较无统计学意义（P〉0.05）。D组有1例发生运动阻滞。[结论]0.1%~0.15%罗哌卡因＋0.2μg.mL-1舒芬太尼能够为产妇提供安全可靠的术后镇痛。[Objective] To observe the different concentration of ropivacaine combining sufentanil for postcaesarean analgesia.[Methods] One hundred twenty women were enrolled in the study.They were randomly assigned and divided into four groups.The concentration of ropivacaine was 0.075%（group A）,0.1%（group B）,0.125%（group C） and 0.15%（group D）,combined by the sufentanil 0.2μg/ml.Pain（Visual Analog Scale）,motor blockage（Bromage scale） and side effects were recorded.[Results] The VAS score of group A was significantly higher than groups B,C,D.There was no significant difference of the VAS score between groups B,C and D.But the bromage score of two patients were higher in group D.[Conclusion] 0.1%～0.15% Ropivacaine combining 0.2μg·mL（-1）sufentanil was effective and safe for postcaesarean analgesia.
