Therapeutic efficacy of Interferonα-2b on rat liver fibrosis evaluated by diffusion-weighted MR imaging

摘  要:目的:探讨MRI表观弥散系数(ADC)值评价干扰素α-2b(INFα-2b)干预四氯化碳(CCl_4)诱导的大鼠肝纤维化疗效的价值。方法:将45只SD雄性大鼠随机分为正常对照组、干扰素治疗组、肝纤维化模型组3组,每组15只。正常对照组每只大鼠给予0.02mL/kg生理盐水腹腔注射。肝纤维化模型组每只大鼠给予40%CCl_4(CCl_4:橄榄油=2:3)0.02mL/kg腹腔注射,每周2次,共8周。干扰素治疗组每只大鼠给予40%CCl_4 0.02mL/kg腹腔注射,每周2次,共8周。第5周起,干扰素治疗组大鼠每周3次给予腹腔注射INFα-2b 1×10~6单位/只,至第8周。于第8周末对3组大鼠进行MRI常规和弥散加权成像(DWI)扫描,b取500、700、1000s/mm^2时,比较DWI图像的信号强度和ADC值的变化。对3组肝纤维化大鼠病理变化及α平滑肌肌动蛋白(α-SMA)的表达和ADC值进行比较分析。结果:b=500s/mm^2时,正常对照组、肝纤维化模型组、干扰素治疗组3组的ADC值分别为(3.63±0.21)×10^(-3)mm^2/s、(1.17±0.11)×10^(-3)mm^2/s、(2.82±0.06)×10^(-3)mm^2/s;b=700s/mm^2时,3组ADC值分别为(3.12±0.68)×10^(-3)mm^2/s、(1.60±0.42)×10^(-3)mm^2/s、(2.15±0.35)×10^(-3)mm^2/s;b=1000s/mm^2时,3组ADC值分别为(2.16±0.93)×10^(-3)mm^2/s、(1.31±0.06)×10^(-3)mm^2/s、(1.45±0.53)×10^(-3)mm^2/s。b=500s/mm^2时,干扰素治疗组ADC值大于肝纤维化模型组而小于正常对照组(P<0.05)。HE染色显示,干扰素治疗组大鼠的严重肝纤维化比例明显低于肝纤维化模型组,但高于正常对照组(P均<0.05);而其肝纤维化标志物α-SMA亦低于肝纤维化模型组,但高于正常对照组(P均<0.05)。结论:MRI的ADC值可评价INFα-2b干预肝纤维化的效果。Objective To investigate the value of diffusion-weighted MR imaging for evaluating the therapeutic efficacy of Interferonα-2b on rat liver fibrosis induced by CCl_4.Methods Forty-five male SD rats were randomly divided into 3 groups.Rats in normal control group(n=15) received sodium chloride intraperitoneally at the dose of 0.02 mL/kg. Rats in liver fibrosis model group(n=15) received 40%CCl_4(CCl_4:oliver oil = 2:3) intraperitoneally at the dose of 0.02 mL/kg twice per week until the eighth week.Rats in INFα-2b treatment group(n=15) were treated with the same protocol as rats in liver fibrosis model group and received intraperitoneal injection of INFα-2b three times per week since the fifth week. MR scans and MR-DWI with different b values(500,700,1000 s/mm^2) were performed in all three groups at the end of 8th week.The stage of liver fibrosis,expression ofα-SMA and ADC between the three groups were compared.Results When b values were to 500,700,and 1000 s/mm^2,ADC values in normal control group were(3.63±0.21 )×10^(-3)mm^2/ s, (3.12±0.68)×10^(-3)mm^2/ s,and(2.16±0.93)×10^(-3)mm^2/s,respectively;ADC values in liver fibrosis model group were(1.17±0.11)×10^(-3)mm^2/s,(1.60±0.42)×10^(-3)mm^2/s,and(1.31±0.06)×10^(-3)mm^2/s,respectively;and ADC values in INFα-2b treatment group were(2.82±0.06)×10^(-3)mm^2/s,(2.15±0.35)×10^(-3)mm^2/s,and(1.45±0.53)×10^(-3)mm^2/s,respectively.When b value was 500 s/mm^2,ADC value in INFα-2b treatment group was higher than that in liver fibrosis model group,but lower than that in normal control group(P0.05).HE staining showed that liver fibrosis staging in INFα-2b treatment group was lower than that in liver fibrosis model group but higher than that in normal control group(P0.05).The expression ofα-SMA in INFα-2b treatment group was lower than that in liver fibrosis model group(P0.05)but higher than that in normal control group.Conclusions Diffusion-wei
