摘  要:目的:研究冠脉搭桥术(CABG)中直视下心肌内注射移植骨髓单个核细胞治疗缺血性心脏病的可行性和疗效。方法:入选经冠状动脉造影确诊的冠心病患者24例,随机分为骨髓干细胞移植组(12例)和对照组(12例)。移植组除常规手术治疗外,同期术中将骨髓单个核细胞悬液分多点注射到缺血心肌内。对照组仅采用常规手术治疗。用经胸超声心动图和单光子放射计算机断层显像术(SPECT)比较两组治疗前、后超声指标和心肌灌注缺损区面积。结果:术后随访1年,经胸超声心动图显示,与术前及对照组术后相比,移植组左室射血分数等指标明显改善(P<0.05);SPECT显示,移植组心肌灌注缺损面积明显降低(P<0.05)。移植组在移植术中和术后均无并发症发生。结论:冠脉搭桥术中心肌内注射移植骨髓干细胞治疗缺血性心脏病安全可行,术后能改善左室收缩功能及心肌血流灌注。Objective: To study the feasibility and effectiveness of myocardial injection of transplanted bone marrow mononuclear cells(BMMCs) in the treatment of ischemic heart disease during coronary artery bypass grafting(CABG).Methods: The selected 24 coronary heart disease patients confirmed by coronary angiography were randomly divided into BMMCs transplantation group(12 cases) and conventional surgical treatment(control group,12 cases).The left ventricular ejection fraction(LVEF) and the size of myocardial perfusion defect were measured by echocardiography and single-photon-emission computed tomography(SPECT) respectively before and after the treatment.Results: The one-year follow-up results showed that,compared with control group,the transplantation group had significantly increased LVEF measured by transthoratic echocardiogram and had significantly decreased myocardial perfusion defect area measured by SPECT(both P0.05).There were no complications in the transplantation group during and after transplantation operation.Conclusion: Myocardial injection of transplanted BMMCs in CABG for treating ischemic heart disease improved the left ventricular systolic function and myocardial perfusion,and it is safe and feasible for clinical application.
