无症状过敏者外周血单个核细胞存在过敏原刺激后的高水平IL-10反应

摘　　要：目的:测定无症状的过敏者外周血单个核细胞(PBMC)经特异性过敏原刺激后CD4+IL-10+调节性T细胞数量和IL-10水平,并比较其与过敏性哮喘患者和不过敏健康人的差异。方法:选取无症状的尘螨过敏者24例,急性发作期的尘螨过敏性哮喘患者20例,不过敏的健康人22例,均分离PBMC并CFSE染色,分别在不刺激和1mg/L尘螨抗原(rDer p 1)刺激情况下体外培养7d。流式细胞仪检测各组CD4+IL-10+T细胞/CD4+细胞百分比,分析CD4+IL-10+T细胞分裂指数;ELISA法检测各组细胞培养上清液中IL-10和TGF-β1的水平。结果:①rDer p 1刺激后无症状的尘螨过敏者CD4+IL-10+T细胞/CD4+细胞百分比高于过敏性哮喘患者;无论刺激与否,无症状的尘螨过敏者CD4+IL-10+T细胞/CD4+细胞百分比与不过敏的健康人水平接近。②与不过敏的健康人和过敏性哮喘患者比较,无症状的尘螨过敏者表现为rDer p 1刺激后PBMC高水平的IL-10反应。结论:无症状过敏的发生可能与机体对特异性过敏原刺激产生高水平的IL-10反应有关。Objective： To determine the baseline levels and the responses of CD4＋ IL-10＋ regulatory T cells and IL-10 after allergen exposure in peripheral blood mononuclear cells（PBMCs） derived from asymtomatic atopic people.Methods： PBMCs were isolated from 24 subjects sensitized to house dust mite asymptomatic,20 patients sensitized to house dust mite with asthma,and 22 non-allergic subjects.Cells were cultured without stimulation and with 1 mg/L rDer p 1.CD4＋IL-10＋ T cells/ CD4＋ T cells percent were measured by flow cytometry,division index of CD4＋IL-10＋ T cells was analyzed after CFSE stain.Concentrations of IL-10 and TGF-β1 in culture supernatants were determined by ELISA.Results： ①PBMCs derived from sensitized asymptomatic subjects had higher percentage of CD4＋IL-10＋ T cells than those from sensitized asthmatic patients in rDer p 1-stimulated cultures.CD4＋IL-10＋ T cells/CD4＋ T cells percent in sensitized asymptomatic group were similar to those in non-allergic group both in un-stimulated and rDer p 1-stimulated cultures.②Sensitized asymptomatic group had the highest level of IL-10 response to allergen among the three groups.Conclusion： The high level of IL-10 responses to specific allergen may be associated with the development of sensitized asymptomatics.
