T2DM患者UAE与vWF：Ag及血小板参数的相关性研究

摘　　要：目的 探讨2型糖尿病患者尿白蛋白排泄量（UAE）与vWF：Ag、血小板参数的相关性.方法 检测172例T2DM（T2DM组）患者UAE,据24h-UAE水平分为3组：①正常白蛋白尿组（NAU）组,②微量白蛋白尿（MAU）组,③临床白蛋白尿（CAU）组.选取60例健康人作为健康对照（NC）组.同时检测各组对象的血浆vWF：Ag水平、血小板计数（PC）、平均血小板体积（MPV）、血小板分布宽度（PDW）.结果 T2DM组患者血浆vWF：Ag、PC水平均明显高于NC组（P〈0.01）,MAU组和CAU组患者UAE,PC,MPV,PDW明显高于NC组（P〈0.01）.多元逐步回归分析表明,T2DM组的UAE及血浆vWF：Ag,PC,MPV,PDW水平与NC组存在明显正相关（r分别为0.829,0.717,0.576,0.632,P值均〈0.01）;T2DM患者的UAE与血浆vWF：Ag,PC,MPV,PDW水平存在明显的正相关性（r分别为0.804,0.678,0.493,0.547,P值均〈0.01）.T2DM患者的血浆vWF：Ag水平与PC,MPV,PDW呈正相关（r分别为0.858,0.784,0.820,P均〈0.01）,与性别无明显相关性（r=0.391,P〉0.05）.结论 T2DM患者UAE与血浆vWF：Ag,PC,MPV,PDW水平呈正相关,随着UAE的增高,血浆vWF：Ag,PC,MPV,PDW水平也均逐渐升高.UAE,vWF：Ag及血小板参数对糖尿病肾病的早期诊断、血管内皮细胞损伤程度、血液高凝状态有重要评价意义.Objective To investigate the relationship between urinary albumin excretion（UAE） ,platelet parameters,and plasma levels of yon Willebrand Factor Anti-gen （ vWF： Ag） ira type 2 diabetic patients （ T2 DM ）. Methods One hundred and seventy twn T, DM patients and 60 heahhy persons were enrolled in this study. Patients were classified into 3 groups according to their 24h-UAE：normal albuminuria （ NAU ） , microalbuminuria { MAU ） and clinical albmninuria （ CAU ）. The plasma levels of vWF ： Ag, platelets count （ PC ） , mean platelet volumes and platelet distributing width （ PDW ） were measured in each patient. Results The plasma levels of vWF： Ag and PC in diabetic, groups were significantly higher than those in eontrol grnup（ P 〈 0.01 ）. The UAE,PC,MPV and PDW in M AU as well as CAU groups were significantly higher than those in control group（ P 〈 0.01 ）. Multiple gradual regression analysis showed that there was obvious colTelation between the UAE, the vWF ： Ag level, PC, MPV, and PDW of the T2 DM as wll as those of control group （ r = 0. 829,0.717,0.576,0. 632, P 〈 0. 01） ,there was obvious correlation between the UAE and the vWF：Ag level, PC, MPV, and PDW of the Te DM（r = O. 804,0. 678,0. 493,0. 547, P 〈 0.01 ）. There was obvious correlation in T2 DM between the vWF ： Ag level and PC, MPV, and PDW （ r = 0. 858,0. 784,0. 820, P 〈 0. 01 ）. But there was no correlation in T_~ DM between the vWF ： Ag level and the gender（ r = 0. 391 ,P 〉 O. 05 ）. Conclusion There were obvious correlation between the UAE and the vWF ： Ag, PC, MPV, PDW level of the T, DM, and the plasma levels of vWF ： Ag, PC, MPV, PDW were increased as the increase of UAE. The determination of UAE,the plasma levels of vWF ： Ag and the platelet parameters could play an important role in early diagnosis of diabetic nephrnpaihy,evaluation of the injury or coagulation and dysfunction of the generalised vascular endothelial cell.
