RA患者血清VEGF水平变化及与腕关节滑膜血流参数的相关性研究

摘　　要：目的研究超声检测类风湿关节炎(RA)患者腕关节滑膜血流参数结合血清血管内皮细胞生长因子(VEGF)水平检测判断疾病活动性的可行性。方法研究对象为62例治疗前RA患者(RA组)和30例健康志愿者(对照组),两组均采用ELISA法检测血清VEGF水平,RA组同时采用超声检查双侧腕关节滑膜血流分级、滑膜动脉阻力指数(RI);采用Spearman法对滑膜血流分级、RI与血清VEGF水平的相关性进行分析。结果 RA组血清VEGF水平明显高于对照组(P<0.01);RA组滑膜血流分级及RI分别与血清VEGF水平呈显著正相关和负相关r,=0.903、-0.817(P均<0.01)。结论超声检测RA患者腕关节滑膜血流参数结合血清VEGF水平检测可反映疾病活动性,此对RA的临床诊治及疗效评估、预后判断具有重要指导意义。Objective To explore the feasibility of disease activity of synovial blood flow parameters of wrists and serum VEGF level in rheumatoid arthritis（RA） by ultrasound. Methods Research objects were 62 patients with RA before treatment （RA group） and 30 healthy volunteers （control group）, the serum VEGF was detected by ELISA in two groups, RA group was inspected synovial blood flow degree and synovial artery resistance index （RI） of bilateral wrists. The correla- tion between the synovial blood flow degree, arterial RI and the level of VEGF were analyzed by Spearman method. Results Serum VEGF level in RA group was significantly higher than that in control group（ P 〈 0.01 ）. Synovial blood flow degree, RI with the level of serum VEGF had significantly positive and negative correlation respectively, r = 0. 903, - 0.817 （ both P 〈 0.01 ）. Conclusion The combination of synovial blood flow parameters by ultrasound and the level of serum VEGF can pre- dict the activity of RA , and it is helpful for the selections of clinical treatment and evaluating the prognosis.
