摘 要:目的探讨重组乙型肝炎(乙肝)疫苗(酿酒酵母)(Hepatitis B Vaccine Made by Recombinant DeoxyribonucleicAcid Techniques in Saccharomyces Cerevisiae Yeast,HepB-SCY)对新生儿的免疫效果和持久性。方法采用分层随机抽样方法,抽取上海市4个区1997~2008年出生的、有全程HepB-SCY接种史的儿童2356人作为调查对象,按免疫后不同时间分12个组,进行问卷调查,并采集静脉血进行抗乙肝病毒表面抗原抗体[Antibody toHepatitis B Virus(HBV)Surface Antigen(HBsAg),Anti-HBs]定量、HBsAg与抗乙肝病毒核心抗原抗体(Antibodyto HBV Core Antigen,Anti-HBc)检测。结果 Anti-HBs阳性率和几何平均浓度均随免疫时间延长而下降,免疫后1年分别为92.44%和392.61毫国际单位/毫升(mIU/ml),12年分别降至32.19%和55.52mIU/ml。调查对象的平均HBsAg阳性率为0.25%,Anti-HBc阳性率为1.61%,未出现随免疫后时间延长而上升的趋势。疫苗的保护率为88.25%。多因素Logistic回归分析显示:性别、年龄组、母亲怀孕时HBsAg状况、共用毛巾,是儿童乙肝保护性抗体应答的影响因素。结论新生儿使用HepB-SCY免疫有较好的持久性和保护效果,免疫后12年无需加强免疫。Objective To explore the immunity effects and persistence of hepatitis B vaccine made by recombinant deoxyribonucleic acid techniques in saccharomyces cerevisiae yeast(HepB-SCY)on neonates.Methods Using stratified random sampling,2356 children born in 1997-2008 in 4 districts of Shanghai who were vaccinated 3 dosages of HepB-SCY after birth were selected as subjects and were divided into 12 groups according to vaccination time.Questionnaires investigation were conducted and blood samples were collected to test antibody of hepatitis B virus(HBV)surface antigen(HBsAg)(anti-HBs),HBsAg and antibody to HBV core antigen(anti-HBc).Results The positive rate of anti-HBs and geometric mean concentration declined along the extension of vaccintion times with 92.44% and 392.61 mIU/ml respectively at 1 year after vaccination,and 32.19%and 55.52 mIU/ml respectively at 12 years after vaccination.The average positive rate of HBsAg and anti-HBc of all subjects were 0.25%and 1.61% respectively and that didn't show the tendency of rising along with the extension of vaccination times.The protective rate of vaccine was 88.25%.The multivariate Logistic regression showed that gender,age,HBsAg status of mother during pregnancy and share towels were influences of protective antibody response of children.Conclusion HepB-SCY had good persistence and protective effect for neonates and did not need booster after 12 years of primary immunization.
