芬太尼与地佐辛在老年骨科病人镇痛的同时对老年病人循环和呼吸功能的影响的比较

摘　　要：[目的]为降低不良反应发生率,探索老年骨科病人术后新型镇痛药物。[方法]选择2009年1月~2010年12月某院行骨折手术的100例ASAI或Ⅱ级老年患者。单盲随机将其中50例分为芬太尼组组,余者分为地佐辛组。[结果]芬太尼组和地佐辛组镇痛期间,术后2 h呼吸抑制者、低血压眩晕者及心动过缓者构成比对照显示,无明显差异性（P﹤0.05）;术后4 h呼吸抑制者、低血压眩晕者及心动过缓者构成比对照显示,差异有统计学意义（P﹤0.05）。镇痛效果对照显示,术后2 h、4 h视觉模拟（VAS）评分无显著差异性（P﹤0.05）。术后2 h心率（HR）、平均动脉压（MAP）、呼吸频率（RR）及血氧饱和度平均值对照显示,无明显差异性（P﹤0.05）;术后4 h心率（HR）、平均动脉压（MAP）、呼吸频率（RR）及血氧饱和度平均值对照显示,差异有统计学意义（P﹤0.05）。[结论]在老年骨科病人术后镇痛中,发挥相同镇痛的同时,地佐辛对老年病人循环和呼吸功能的影响显著低于芬太尼,提示地佐辛可作为骨科术后镇痛的新型替代药物。[Objective]To reduce the incidence of adverse reactions,to explore the novel analgesic drugs of elderly orthopaedic patients postoperatively.[Methods]Selected 100 patients with ASAI or II level in elderly patients with fracture operation from 2009 January-2010 December.A randomized single blind method was used,the patients were divided into fentanyl group and dezocine group.[Results]Fentanyl group and dezocine group during epidural analgesia,postoperative 2h respiratory depression,hypotension vertigo and bradycardia were not significantly different（P﹥0.05）between two groups;4h postoperative respiratory depression,hypotension,dizziness and bradycardia were not significantly different（P﹤0.05）between two groups.Analgesic effect after 2h surgery,4h visual simulation（VAS）score were not significantly different（P﹥0.05）between two groups.Postoperative 2h heart rate（HR）,mean arterial pressure（MAP）,respiratory rate（RR）and oxygen saturation were not significantly different（P﹥0.05）between two groups;4h postoperative heart rate（HR）,mean arterial pressure（MAP）,respiratory rate（RR）and oxygen saturation mean value were with significant differences（P﹤0.05）between two groups.[Conclusion]In postoperative analgesia of elderly orthopedic patients,dezocine and fentanyl play the same analgesia effect,while the effect of the former on circulatory and respiratory function of elderly patients is significantly lower than that of fentanyl,which suggests dezocine can be the new alternative medicine when postoperative analgesia after orthopedic surgery.
