周围血清P0蛋白抗体表达对快速进展性自身免疫性内耳病和梅尼埃病的诊断价值

摘　　要：目的探讨周围血清P0蛋白(myelin protein zero,MPZ)抗体水平对快速进展性自身免疫性内耳病(autoimmune inner ear diseases,AIED)和梅尼埃病的诊断价值。方法 40例AIED患者(研究组)包括快速进展性AIED 14例、梅尼埃病11例和遗传性聋15例,通过酶联免疫标记法,分别定量检测患者周围血清MPZ抗体IgG和IgM浓度,结果与对照组(其他自身免疫性疾病40例,听力损伤者除外)比较。结果研究组周围血清MPZ抗体浓度(IgG:61.53±13.55ng/L和IgM:60.34±13.96ng/L)明显高于对照组(IgG:30.77±9.80ng/L和IgM:20.82±12.29ng/L)(P<0.01),MPZ抗体浓度由高至低依次为快速进展性AIED、梅尼埃病、遗传性聋,双侧梅尼埃病患者周围血清MPZ抗体浓度接近快速渐进性AIED患者(P>0.05),检测IgM浓度更具临床诊断意义。结论周围血清MPZ抗体的表达特别是IgM的表达,在诊断快速进展性AIED与双侧梅尼埃病方面,有较高的敏感性和特异性。Objective To investigate the diagnostic value of myelin protein zero（P0,MPZ） protein in rapidly progressive autoimmune inner ear diseases（AIED） and Ménière disease.Methods Forty patients with AIED were included in this prospective study：patients with rapidly progressive AIED,or with M ni re disease,or with congenital hearing loss.We detected the density of MPZ in blood serum by ELISA,and controlled to group corresponding with patients with others autoimmune diseases（exclude inner ear disease）.Results The densities of MPZ in blood serum in study group（IgG：61.53±13.55 ng/L and IgM：60.34±13.96 ng/L） were significantly higher than those in control group（IgG：30.77±9.80 ng/L and IgM：20.82±12.29 ng/L）（P0.01）.The expression of MPZ followed the tendency：rapidly progressive AIEDMénière diseasecongenital hearing loss.The result in bilateral Meniere disease was similar to that in rapidly progressive AIED（P0.05）.Conclusion Detection of MPZ in blood serum,especially the expression of IgM,is useful to diagnose rapidly progressive autoimmune inner ear disease and Meniere disease.
