摘  要:目的:探讨经皮测胆红素在早期新生儿高胆红素血症中的临床价值。方法:随机选择50例(日龄≤8 d)出现黄疸的新生儿。入院时即对患儿父母进行新生儿高胆红素血症及经皮测胆红素的宣教,同时经皮及静脉采血测量TCB和血清胆红素值,用统计学方法分析其相关性。结果:经皮测胆红素与血胆红素之间有良好的相关性(r=0.973,P<0.01)。经皮测胆红素法诊断新生儿高胆红素血症的灵敏度为81.82%,特异度为96.43%。漏诊率为18.18%,误诊率为3.57%,阳性似然比为22.91,阴性似然比为0.19。TCB动态测量对预防新生儿早期胆红素脑病的发生具有很高的临床应用价值。结论:经皮测胆红素可以准确地反映患儿血清胆红素水平,临床可用以监测患儿胆红素水平及治疗效果,其操作及护理简单易行、无创、患儿父母容易接受。Objective:The clinical value of the transcutaneous bilirubin measurement in neonatal hyperbilirubinemia were explored.Methods:50 jaundice patients(≤8 d)were randomly selected,the bilirubin level was measured by transcutaneous bilirubinometry and venous blood respectively.We confered its content and analyzed its correlation using statistical methods.Then,we analyzed the feasibility of monitoring、treatment、preventing bilirubin encephalopathy using TCB.Results:There was a positive correlation between transcutaneous bilirubin and venous blood(r=0.973,P0.01).The sensitivity was 81.82%,specificity was 96.43%,false negative rate was 18.18%,false positive rate was 3.57%,positive likelihood ratio was 22.91,negative likelihood ratio was 0.19 of diagnosing neonatal hyperbilirubinemia with the transcutaneous bilirubinometry.There was a high value in preventing bilirubin encephalopathy with dynamic monitoring Transcutaneous bilirubin.Conclusion:There was a positive correlation between transcutaneous bilirubin and venous blood.Transcutaneous bilirubin is a convenient、fast、accurate method for screening detection,sequent observation,following up,review and determing the effect of treatment on neonatal jaundice.
