液基细胞学检查、阴道镜联合宫颈管搔刮术在宫颈病变诊断中的价值

摘　　要：目的:探讨液基细胞学检查、阴道镜联合宫颈管搔刮术对宫颈病变筛查的临床价值。方法:收集2008-09～2010-09就诊的3900例病人,行液基细胞学检查。对TCT阳性702例病人行阴道镜下宫颈活组织病理检查联合宫颈管搔刮术。结果:TCT诊断阳性702例,阳性率18%,TCT检查与病理学检查总符合率70.65%;阴道镜检查阳性360例,组织学诊断阳性420例,阴道镜检查与病理学检查总符合率59.45%;宫颈管搔刮术阳性30例,结合阴道镜检查阳性率提高不明显。结论:应用TCT筛查宫颈病变,细胞学阳性病例联合阴道镜检查及镜下宫颈活组织病理,对于早期发现宫颈病变具有重要意义,对于阴道镜检查不满意及临床可疑病例联合宫颈管搔刮术仍然具有重要意义。Objective： To evaluate the three techniques,liquid-based thin layer cytology test,colposcopy and endocervical curettage in the clinical screening of cervical lesions.Methods： 3900 patients were screened by liquid-based thin layer cytology test（TCT） between Sept.2008 and Sept.2010.The positive cases were further examined with colposcope and endocervical curettage.Results： 702 cases were identified with abnormal squamous by TCT,the positive rate was 18%.Among them,360 cases were abnormal with colposcopy and 420 with histology;30 cases showed abnormal by endocervical curettage only.Conclusions： The combination of liquid-based thin layer cytology test and colposcope is important in detecting the cervical lesions;still the endocervical curettage has potential feasibility for the unsatisfied colposcope and suspected clinical patients.
