摘  要:目的运用速度向量成像(VVI)技术分析冠心病患者左室缺血心肌运动功能。方法选择正常对照组20例及冠心病组40例,其中冠心病组又按照冠脉造影结果分为心肌缺血组(14例)和心肌梗死组(26例)。VVI技术测量左心室心肌纵向及径向收缩期峰值速度(Vs)、应变(SRs)及应变率(ε),比较冠心病组和对照组之间各节段有无差异。结果 (1)梗死节段纵向及径向Vs、SRs、ε明显低于对照组相应节段。(2)缺血节段纵向Vs、SRs、ε较正常组相应节段减低,较冠脉造影相比VVI诊断前间隔基底段心肌缺血的敏感性为85.7%,特异性为88%。结论 VVI技术较冠脉造影方便、快捷、重复性好,有望成为临床上无创诊断冠心病特别是严重心肌缺血的一项新技术。Objective Using velocity vector imaging to assess left ventricle wall function of longitudinal segmental motion and radial segmental motion in patients with coronary artery disease.Methods Twenty healthy volunteers and forty patients with coronary artery disease were selected,and we divided them into group of normal and of coronary artery disease.Based on the result of coronary angiography,coronary artery group was divided into the myocardial ischemia group(14 patients) and the myocardial infarction group(26 patients).Then we analysed the systolic myocardial maximum velocity,the maximum strain and the maximum systolic strain rate of each group of left ventricle.Through these data we can evaluate whether it is different between group of normal and of coronary artery disease.Results(1)Vs,SRs,ε of longitudinal and radial segment infarction of left ventricle significantly lower than the corresponding segments of the control group.(2)Compared with the corresponding segment,longitudinal Vs,SRs,ε of ischemic segments lower than the normal group,compared with coronary angiography,the diagnostic ensitivity of myocardial infarction by VVI was 85.7%,the specificity was 88%.Conclusion Compare to coronary angiography,VVI is convenient,fast and reproducible,is expected to be a new clinically technology to get non-invasive diagnosis of severe myocardial ischemia in coronary heart disease.
