摘  要:目的分析系统性红斑狼疮患者颈动脉内中膜背向散射积分与动脉弹性功能相关性,探讨该指标在绝经前成年患者早期动脉粥样硬化诊断中的意义。资料与方法根据系统性红斑狼疮活动指数将43名患者分为狼疮活动组(24例)及狼疮非活动组(19例),并选取年龄、性别匹配的15名志愿者作为对照组,比较各组颈动脉常规超声、内中膜背向散射积分及动脉弹性功能等指标,计算后两者相关性。结果狼疮活动组颈动脉内中膜厚于狼疮非活动组及对照组,狼疮活动组及非活动组颈动脉c-IBS、僵硬度及PWV均明显高于对照组(P<0.05),颈动脉c-IBS与僵硬度及脉搏波传播速度(PWV)存在正相关关系。结论颈动脉内中膜背向散射积分与动脉弹性功能相关性良好,可以用于评价系统性红斑狼疮早期动脉粥样硬化。Purpose To assess the relationship of carotid integrated backscatter and arterial stiffness to subclinical vascular atherosclerosis in pre-menopausal adult patients with systemic lupus erythematosus. Materials and Methods Forty-three cases of SLE were divide into active SLE group(SLEDAI≥10, n=24) and stable SLE group(SLEDAI10, n=19), fifteen sex and age matched healthy controls also underwent integrated backscatter and arterial stiffness carotid ultrasound analysis. A correlation analysis was used to determine the independent link between carotid corrected-IBS(c-IBS), arterial stiffness(β) and pulse wave velocity(PWV). Results Patients of active SLE group showed a larger intima-media thickness(IMT). The carotid c-IBS,β and PWV were significantly higher in active and stable SLE group than control group. Compared with stable SLE group, the carotid c-IBS,β and PWV in active SLE group increased significantly (P0.05). Carotid c-IBS was strongly and positively correlated with β (r=0.881, P0.000 1) and PWV(r=0.914, P0.0001). Conclusions Carotid c-IBS was correlated with β and PWV. The carotid integrated backscatter analysis can evaluate subclinical vascular atherosclerosis in pre-menopausal adult patients with systemic lupus erythematosus.
