摘  要:目的应用速度向量成像(VVI)定量分析不同超敏C反应蛋白(hs-CRP)水平尿毒症患者的左心功能,探讨该技术在临床中的应用价值。资料与方法60例尿毒症患者根据hs-CRP水平分为hs-CRP正常组(A组,30例)和hs-CRP升高组(B组,30例),并随机选择30例健康志愿者作为对照组。VVI技术测定左心长轴16个节段的收缩期峰值应变率(SSR)、舒张期峰值应变率(DSR),并进行各组之间的比较。结果与对照组相比,A、B组心肌各节段DSR均减低,差异有统计学意义(P<0.05或P<0.01)。与A组相比,B组各节段DSR均减低,差异有统计学意义(P<0.05);B组SSR均减低,其中前壁及侧壁中间段、心尖段,下壁基底段、中间段、心尖段,后间隔心尖段,前间壁、后壁中间段的SSR差异具有统计学意义(P<0.05)。结论 VVI技术可敏感地反映hs-CRP升高患者左心节段心肌功能的损害较严重。Purpose To investigate the left ventricular function in uremic patients with diffferent quantity of C-reactive protein(hs-CRP) using velocity vector imaging(VVI). Materials and Methods 60 patients with uremia were assigned into to 2 groups based on the value of hs-CRP: group A with high hs-CRP (hs-CRP3mg/L, n=30),group B with normal hs-CRP group(hs-CRP3mg/L, n=30). Normal control group included 30 healthy subjects. Longitudinal systolic maximum strain rate (SSRmax) and diastolic maximum strain rate (DSRmax) were measured using VVI in the left ventricles (LV) of uremic patients and compared to 30 healthy subjects. Results Compared with normal group, DSR of groups A and B patients in all segments were lower(group A,P0.05;group B,P0.01).Compared with Group A, group B patients’ DSR was significantly lower (P0.05); SSRs in middle and apex of anterior wall and lateral wall,base、middle and apex of superior wall,apex of posterior septum,middle of anterior septum and posterior wall were significantly lower(P0.05). Conclusion VVI is a sensitive tool to assess LV segmental longitudinal function in patients with different levels of hs-CRP.
