摘 要:目的探讨在体质量指数(BMI)正常的蒙古族人群中炎症标志物水平与高血压的关系。方法采用横断面研究的方法,纳入BMI正常的内蒙古通辽市的居民1784人为研究对象,收集各研究对象的基线特征信息,按照血压水平将研究对象分为高血压组(n=583)和血压正常组(n=1201),采集两组研究对象血样并进行血管紧张素Ⅱ(AngⅡ)、高敏C反应蛋白(hsCRP)、可溶性细胞间黏附分子1(sICAM-1)、可溶性E选择素(sE-selectin)的检测。采用协方差分析方法研究炎症标志物水平与血压的关系,采用logistic回归模型分析炎症标志物水平与高血压的关联。结果高血压组的AngⅡ、hsCRP、sICAM-1、sE-selectin水平均高于血压正常组[分别为52.0(40.8,85.9)比47.0(39.3,65.8)ng/L,6.7(4.4,13.7)比5.3(3.5,9.2)mg/L,(334.3±98.0)比(320.1±95.4)μg/L,18.5(15.0,24.4)比17.9(14.1,23.4)μg/L,均P<0.05]。协方差分析结果显示,收缩压与舒张压均随AngⅡ和hsCRP水平增加而升高,线性趋势性检验具有统计学意义。多因素Logistic回归分析表明,AngⅡ,hsCRP水平每升高1个单位,研究对象发生高血压的危险性分别增加至1.005,1.018倍。结论在BMI正常的蒙古族人群中,AngⅡ、hsCRP水平与血压以及高血压相关联。Objective To explore the association between inflammatory biomarkers and hypertension among Inner Mongolians without overweight or obesity. Methods In this cross-sectional study, 1784 Mongolians with normal body mass indexes were recruited, Baseline characteristics were collected. Subjects were divided into hyperten- sion group(n= 583 ) and normotension group (n =1201 ) . Angiotensin Ⅱ ( Ang Ⅱ ), high-sensitivity C-reactive protein (hsCRP), soluble intracellular adhesion molecule 1 (slCAM-1) and soluble E-selectin (sE-selectin) were examined. Covariance analysis was used to examine the relationship between inflammation biomarkers and systolic blood pressure and diastolic blood pressure. The odds ratios of hypertension were calculated using nonconditional logistic regression models. Results Compared with normotension group, the plasma level of Ang [I , hsCRP, sICAM-1 and sE-selectin were all significantly higher in hypertension group [52.0(40. 8,85.9) vs 47.0(39.3, 65.8)ng/L, 6.7(4.4, 13.7) vs 5.3(3.5,9.2)mg/L, (334.32=98.0) vs {320.1+95.4)μg/L, 18.5(15.0,24.4) vs 17.9(14.1,23.4)μg/L, respectively, all P〈0.05]. After adjusting the age, gender, smoking, drinking, waist circumference and hypertension family history, covariance analysis showed that mean levels of systolic and diastolic blood pressures increased with the increasing Ang Ⅱ and hsCRP. Multivariate logistic regression showed that odds ratio of hypertension were 1. 005 for Ang Ⅱ and 1. 018 for hsCRP. Conclusion Elevated plasma levels of Ang Ⅱ and hsCRP are associeted with the increase of blood pressure in Inner Mongolians with normal body mass index.
