Double Locking Plate Through Approach of Extra-articular Oblique Osteotomy of Olecranon in Patients with Distal Humerus Fractures

摘　　要：目的 探讨经尺骨鹰嘴关节外截骨入路应用双锁定钢板治疗成人肱骨远端骨折的临床疗效.方法 收集2008年8月～2010年2月期间成人肱骨远端骨折病例资料44份,经尺骨鹰嘴关节外截骨入路和肱三头肌舌形肌瓣入路应用双锁定钢板内固定.术后随访观察远期疗效,根据肘关节改良Cassebaum评分系统评定肘关节术后远期疗效并对两种入路进行对比.结果 双锁定钢板肱三头肌舌形肌瓣入路肘关节术后远期疗效评分为：优4例,良6例,中4例,差3例,优良率为58.80%;双锁定钢板尺骨鹰嘴关节外截骨入路肘关节术后远期疗效评分为优15例,良9例,中2例,差1例,优良率为88.89%.经χ2检验,疗效差异有显著性（P〈0.05）.结论 经尺骨鹰嘴关节外截骨入路,应用双锁定钢板治疗肱骨远端骨折,手术方法简单、可靠、并发症少、骨折固定牢靠,能早期进行肘关节功能锻炼,取得理想疗效.Objective To explore the clinical efficacy of double locking plate through approach of extra-articular oblique osteotomy of olecranon in adult patients with distal humerus fractures. Methods Forty four cases were chosen from the surgical data of adult distal humerus fractures in our hospital from August 2008 to Febmal.v 2010. They were treated through approach of extra-articular oblique osteotomy of olecranon and triceps tongue-shaped flap, and the hmg term effect was obselved and evaluated according to the elbow modified eassebaum scoring system. Results The score for the long term effect of elbow treatment through triceps tongue-shaped flap approach ：with double locking plate 4 cases were excellent,6 cases were good,4 cases were moderate and 3 cases were bad,so the fineness rate was 58.80%. In the group of extra-articular oblique osteotomy of oleeranon approach,with double locking plate 15 cases were excellent,9 cases were good,2 cases were moderate and 1 case was bad, so the fineness rate was 88.89%. The difference between the two groups was significant analyzed by x^2 test （ P 〈 0.05 ）. Conclusion Internal fixation with double locking plate through approach of extra-articular oblique osteotomy of olecranon in adult patients with distal humerus fractures has many advantages. It provides satisfactory stability with simple technical procedure and solid fixation,It facilitates rehabilitation exercises and provides good results with low complication rate.
