摘  要:目的:探讨双源CT(dual-sourcecomputertomography,DSCT)副鼻窦成像技术的最优剂量及其临床应用的可行性。方法:前瞻性纳入接受副鼻窦检查的成人患者120例,将其随机分为六组,每组20例。分别采用不同管电压和(或)管电流对不同组病例进行副鼻窦DSCT成像,第1组采用120kV,200mAs;第2组采用120kV,100mAs;第3组采用120kV,50mAs;第4组采用100kV,100mAs;第5组采用100kV,50mAs;第6组采用80kV。100mAs。由2名影像科副主任医师以双盲法对所有副鼻窦重建图像进行图像质量评分并取其平均值,对各组的得分情况进行单因素方差分析。由2名主任医师以双盲法将所有重建图像分为以下3等:①无明显伪影的优质图像;②少许伪影,但不影响诊断;③图像质量不能满足诊断要求。对不同组重建图像的质量等级行x2检验。结果:第1~6组患者所受的辐射剂量依次为24.97、12.48、6.24、7.49、3.74、3.48mGy。第1~5组图像质量评分差异无统计学意义(P〉0.05):第6组图像质量评分最低,与其他各组比较差异有统计学意义(P〈0.05)。第1~5组重建图像均能满足诊断要求,第6组有16例(16/20)的重建图像能满足诊断要求。结论:在副鼻窦DSCT检查中采用恰当的管电流及管电压成像.可以保证图像质量并显著降低患者辐射剂量。Objective: To discuss the optimal dose of paranasal sinuses imaging with dual-source computer tomography (DSCT) and its feasibility for clinical application. Methods: 120 patients with the paranasal sinuses scanned using DSCT were prospectively enrolled in this study. These patients were randomly divided into 6 groups, 20 cases in each group. During the paranasal sinuses imaging with DSCT, different radiation doses were applied in each group, Group 1:120 kV, 200 mAs; group 2:120 kV, 100 mAs; group 3:120 kV, 50 mAs; group 4:100 kV, 100 mAs; group 5:100 kV, 50 mAs; group 6:50 kV, 100 mAs. The scores of all reconstructed DSCT images of paranasal sinuses were given by two double blind associate chief physicians of the imaging department. The average scores of every image in each group were compared with one-factor analysis of variance. All reconstructed DSCT images of paranasal sinuses were classified into 3 groups by two chief physi- cians of the imaging department: (1) excellent images without artifacts; (2) a few artifacts with no effects on diagnosis; (3) images with lower quality that could not satisfy the requirement of diagnosis. Chi-square test was applied to compare the grades of the image quality between each group. Results: For the groups 1 to 6, the radiation dose was 24.97, 12.48, 6.24, 7.49, 3.74 and 3.48 mGy respectively. The scores had no statistical significant difference in the groups 1-5 (P〉0.05). The scores of the group 6 were the lowest, and the statistical significance was found when compared with other groups. For the groups I to 5, the images quality could satisfy the requirement of diagnosis. In group 6, there were 16 eases (16/20) with sound images quality that were satisfy the requirement of diagnosis. Conclusion: During the paranasal sinuses imaging with DSCT, optimal voltage and current can be used for cutting down the radiation dose and guaranteeing the images quality.
