摘 要:目的探讨应变率成像(SRI)技术在高血压患者右室早期舒张功能中的应用价值,为临床早期诊断和治疗提供可靠的依据。方法 50例原发性高血压患者分为高血压Ⅰ组32例(左室心肌正常组)和高血压Ⅱ组18例(左室心肌肥厚组),同时选取健康人30例作为对照组。分别对三组进行传统的脉冲三尖瓣口血流(PW)及瓣环平面4个不同位点的SRI和多普勒组织(DTI)检测并对比分析。结果高血压组三尖瓣环4个不同位点的SRe均降低、SRa均升高,与对照组相比差异具有统计学意义(P<0.05);SRe/SRa<1位点检出率高血压Ⅱ组(心肌肥厚组)>高血压Ⅰ组(心肌正常组)(P<0.05)。高血压组3种检测方法比较,SRI检测SRe/SRa<1的总检出率(87.50%)大于DTI的Em/Am<1(78%)尤其在Ⅰ组中表现更为明显,检出率为SRI>DTI>PW(P<0.05)。结论 SRI优于传统PW和DTI检测法,特别是对高血压患者右室早期舒张功能异常的判断更为敏感、准确。Objective To explore the value of strain rate imaging (SKI) technology in the examination of early right ventricular diastolic function in patient with hypertension, and to provide guidance for the early diagnosis and treatment of the disease. Methods Fifty patients with primary hypertension were divided into Group I (32 patients with normal left ventricular)and Group II (18 patients with left ventricular hypertrophy). Thirty healthy individuals were selected as the control group. All the subjects in the three groups were treated by pulse Doppler (PW), Doppler tissue imaging (DTI) and SKI (performed in four different basal myocardial segments with tricuspid valve). Results In group I and group 11, SRe of the four different basal myocardial segments were all significantly lower than that in the control group (P〈0.05). The SRe/SRa〈I detection rate of Group U was significantly higher than that of Group l, and that of the control group was the lowest (P〈0.05). Total detection rate of SKI (87.50%) was higher than Era/Am〈 1 of DTI (78%) and the E/A 〈1 of PW (54%), and these changes were the most pronounced in Group I. Conclusion SKI can provide more accurate diagnosis for the early right ventricular diastolic dysfunction in patient with hypertension compared with traditional technologies
