IL-6、TNF—α的测定与COPD患者不同营养状态的相关性研究

摘　　要：目的 研究白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）对慢性阻塞性肺疾病（COPD）患者营养状态的影响.方法 选择COPD临床稳定期患者49例,根据体质量指数（BMI）、理想体质量百分比（NW%）、肱三头肌皮褶厚度（TSF）、上臂中部臂围（MAC）、血清白蛋白（ALB）及总淋巴细胞（LYM）等营养参数,分为COPD营养不良组（COPDⅠ组）25例,COPD非营养不良组（COPDⅡ组）24例,正常对照组25例.用酶联免疫吸附实验法（ELISA）测定各组血清IL-6、TNF-α,并分析血清IL-6、TNF-α与各营养参数之间的相关性.结果 COPD组BMI、NW%、MAC、TSF低于对照组,IL-6、TNF-α高于对照组,差异有统计学意义（P〈0.05）;Ⅰ组BMI、NW%、MAC、TSF、LYM均低于Ⅱ组及对照组,差异有统计学意义（P〈0.05）;COPDⅠ组、Ⅱ组IL-6、TNF-α显著高于对照组,差异有统计学意义（P〈0.05）,Ⅰ组、Ⅱ组IL-6与BMI、NW%呈显著负相关.COPDⅠ组血清TNF-α与BMI、NW%呈显著负相关.结论 COPD患者的营养不良与IL-6、TNF-α水平有一定的关系,IL-6、TNF-α增高可能导致COPD患者病情加重和营养状态恶化.Objective To investigate the correlation between the serum interleukin -6 （IL -6 ） levels and tumor necrosis factoe -alpha（ TNF - α） on nutrition state in the patients with chronic obstractive pulmorary disease（ COPD）. Meth-otis Fourty - nine COPD patients in stable stage and twenty - five healthy controls were studied. Nutritonal parameters, in- cluding body mass indox（ BMI）, percentage of normal body mass（ NW% ）, tricepsskin - fold thickness（ TSF）, mid - upper ann circumference（ MAC）, serum albumin（ ACB ）, told lymphocyte couts（LYM） were determined. 25 COPD patients were divided into malnutrition group （ groupⅠ ）, 24 COPD patients non - malnoutrtion group （ group Ⅱ ）. Serum IL - 6 and TNF - α were measured by enzyme -linked immunonsorbent assay（ELISA） at the same time. The correlation between IL -6 and nutritional parameters as well as TNF - （xwas analyzed. Results BMI, NW% , TSF and MAC in COPD patients were sig- nificantly lower than those in healthy controls, while the levels of IL -6 and TNF - （xin cases were significantly higher than in controls （P 〈 0.05 ）. BMI, NW% , MAC, TSF and LYM in group I were significantly lower than that in healthy controls and in group Ⅱ （ P 〈 0. 05 ）. Serum IL - 6 in group I and group Ⅱ showed a negative correlation with BMI and NW%. A signifi- cant negative correlation existed between serum TNF - α levels in group I and results of their BMI, NW%. Conclusion Malnutrin COPD patients IL - 6 and TNF - αtakes part in the inflammation activity and can worse the level of nutrition in patient with COPD.
