摘  要:目的测量单纯肥胖青少年合并脂肪肝减肥前后肝脏、脾脏、肾脏超声背向散射积分的变化,探讨超声背向散射积分在判断脂肪肝程度中的应用价值。方法 60例单纯肥胖青少年,其中男性30例,女性30例;年龄9~24岁,平均年龄14岁;身高(1.61±0.11)m,体质量(76.68±18.36)kg,体质量指数(BMI)29.39±4.73。参加减肥训练营1个月,使用HP-SONOS5500型超声诊断仪,配置AD装置及声学定量分析系统,测量减肥前后肝脏、脾脏、肾脏超声背向散射积分值,同时监测血脂、血糖的变化。结果单纯肥胖青少年减肥前后肝脏和肾脏超声背向散射积分值变化有统计学意义(P<0.05),脾脏背向散射积分值变化无明显统计学意义。结论超声背向散射积分可以用于定量判断脂肪肝的程度,监测脂肪肝的变化,观察治疗效果,其他临床应用有待进一步研究。Objective To examine the changes in liver ultrasonic integrated backscatter(IBS) between obese adolescents with fatty liver before and after a weight loss program,and evaluate the value of ultrasonic IBS in assessing the degrees of fatty liver.Methods A total of 60 obese adolescent were enrolled,male 30 and female 30,aged 9-24 years old,mean age 14 years old.Their mean height was(1.61 ± 0.11) meter,mean weight was(76.68 ± 18.36) kg,and mean body mass index was 29.39 ± 4.73.The IBS parameters of liver,kidney,and spleen of these adolescents,such as average image intensity(AII),standard deviation of image intensity(SDI) and peak to peak intensity(PPI) were measured before and after a weight loss training camp.Blood fat and blood sugar were also monitored.The HP-SONOS 5500 diagnostic ultrasonic equipment with AD technology was used for measurement.Results The IBS changes in liver and kidney before and after the weight loss camp were statistically significant.The IBS change in spleen was not statistically significant.Conclusion It is demonstrated that ultrasonic IBS can evaluate the changes in fatty liver and determine the extent of fatty infiltration.Further research is needed to explore its utility in clinical practice.
