摘 要:目的:观察大黄和早期肠内营养(EEN)对大鼠肠缺血-再灌注损伤(IRI)后肠黏膜屏障的影响。方法:将32只大鼠随机分为对照组、EN组、大黄组、大黄+EN组。在大鼠肠IRI模型上观察再灌注24 h后肠黏膜形态学的变化、血清内毒素和细菌易位指标。结果:肠IRI可导致肠黏膜发生严重损伤。肠IRI后24 h,大黄组大鼠内毒素水平低于EN组(P<0.05)。各组大鼠均出现细菌易位。大黄组和大黄+EN组的细菌易位与对照组和EN组相比均明显降低。绒毛高度/肠腺隐窝深度(V/C)比值的2×2析因分析说明EN和大黄对V/C比值的影响存在协同作用。结论:添加大黄的EN支持治疗能改善大鼠肠黏膜IRI,减轻血清内毒素血症和细菌易位。Obiective: To observe the effect of Rhubarb and early enteral nutrition on intestinal mucosal barrier injury after intestinal ischemia-reperfusion.Methods: Thirty-two Sprague-dowley rats were randomly divided into control,rhubarb,EN and Rhubarb-EN group.A rat model of intestinal ischemia-reperfusion was constructed by clamping the supramesenteric artery.The rats were killed at the 24th hour after reperfusion.Morphologic changes of intestinal mucosa,endotoxin and bacterial translocation in mesenteric tissue,spleen,and liver were determined. Results: After intestinal ischemia-reperfusion,intestinal mucosa was damaged seriously.At the 24th hour after intestinal ischemia-reperfusion,the level of plasma endotoxin in Rhubarb group was lower than that in EN group(P0.05).Bacterial translocation was found in all groups.Bacterial translocation decreased significantly in both Rhubarb group and Rhubarb-EN group compared with both control group and EN group.2×2 factorial ANOVA of V/C ratio showed significant synergistic effect in the interaction of factor Rhubarb and EN.Conclusion: Eeffect of early enteral nutrition added Rhubarb on intestinal mucosal barrier injury after ischemia-reperfusion was benefitial in rats.
