外痔皮瓣整形结合PPH治疗环状混合痔38例临床观察

摘　　要：为探讨外痔皮瓣整形结合吻合器痔上黏膜环切术（PPH）治疗环状混合痔的临床疗效，将76例环状混合痔患者随机分为两组，治疗组38例采用外痔皮瓣整形结合PPH治疗；对照组38例采用外剥内扎术治疗，并对两组临床疗效进行比较分析。结果显示，两组总有效率均为100％，但治疗组在手术时间、住院天数、恢复工作时间方面明显优于对照组（p〈0．05），术后并发症明显低于对照组（P〈0．05）。结果表明，外痔皮瓣整形结合PPH治疗环状混合痔，对肛门功能损伤小、疼痛轻、术后并发症少、恢复快、复发率低、方法简单，具有良好的临床疗效。In order to investigate the clinical effect of the combination surgical therapy of flap-plasty for external hemorrhoids plus PPH in the treatment of circumferential mixed hemorrhoids, authors divided randomly the 76 cases of this disease into two groups：treatment group subjected to above combination therapy （ n =38） ;control group to external dissection internal ligation（ n =38） ;then compared and analysed the therapeutic effects of both groups. As results,the total effective rate of both groups was all 100% ;but in operative time, hospitalization duration, needing time to return work, treatment group was significantly superior to control group （ P 〈0.05）;in incidence of postoperative complication treatment group was significantly lower than control group（ P〈0.05）. Results show that above-mentioned combination therapy for circumferential mixed hemorrhoids is of smaller injury on anal function, milder pain, less postoperative complication, faster recovery, lower recurrence rate and simple manipulation; and has better clinical therapeutic effect.
