右室起搏对左房容积指数及P波离散度的影响

摘　　要：目的比较右室流出道间隔部(RVS)起搏与右室心尖部(RVA)起搏对左房容积指数、房性心律失常及P波离散度的影响。方法选择RVS起搏及RVA起搏各36例,无器质性心脏病,其中DDD起搏30例,VVI起搏6例,行左房容积指数、24h房性心律失常、P波离散度(Pd)及最大P波时限(Pmax)检测,进行对比。结果 36例RVA起搏患者组,14例发生持续性房颤,DDD起搏9例(9/30),VVI起搏5例(5/6);RVS起搏患者组,DDD起搏3例(3/30),VVI起搏3例(3/6),排除持续性房颤患者,RVS起搏左房容积指数、房性心律失常例数、Pd及Pmax分别为:31±7.22ml/m2、8例、32.1±8.2ms及102±9.4ms;RVA起搏左房容积指数、房性心律失常例数、Pd及Pmax分别为:40±8.62ml/m2、18例、50.6±13.2ms及128±13.6ms,两组统计学对比有显著的差异,p<0.05。结论 RVS起搏对房性心律失常的影响优于RVA起搏。Objective To investigate the effects of right ventricular outflow tract septum pacing and right ventricular apical pacing on left atrial volume index,atrial arrhythmia and P wave dispersion.Methods The annual average selected RVS pacing and RVA pacing in 36 cases patients without organic heart disease,including DDD pacing in 30 cases,6 cases of VVI pacing,and measured left atrial volume,24 hours of atrial arrhythmias and P wave dispersion,the use of statistical methods to compare.Results In 36 cases of RVA pacing patients group,14 cases of persistent atrial fibrillation,DDD pacing in 9 cases（9/30）,VVI pacing in 5 cases（5/6）;RVS pacing patients group,DDD pacing in 3 cases（3/30）,VVI pacing in 3 cases（3/6）.Elimination of persistent atrial fibrillation patients,RVS pacing on left atrial volume index,the second group of atrial arrhythmia cases,Pd and Pmax respectively：31±7.22 ml/m2,8 cases,32.1±8.2 and 102ms±9.4ms;RVA pacing on left atrial volume index,the second group of atrial arrhythmia cases,Pd and Pmax respectively：40±8.62 ml/m2,18 cases,50.6±13.2 and 128±13.6ms,the two groups were statistically significant differences in contrast,p0.05.Conclusion Effect of RVS pacing on atrial arrhythmia was better than RVA pacing.
