快速康复外科和传统外科在胃癌围手术期的应用比较

摘　　要：目的比较快速康复外科(FTS)理念和传统外科方法在胃癌围手术期的应用。方法选择胃癌患者82例,围手术期应用快速康复理念42例(下称FTS组),应用传统外科手术40例(下称传统组),将两组手术前后白蛋白检测结果、胃肠恢复时间、住院时间及术后并发症进行比较。结果 FTS组术后白蛋白较对照组水平高,肠道排气时间提前,咽痛、恶心、呕吐发生率较低,住院时间缩短,切口脂肪液化率较高,统计学显示有显著性差异(P<0.05);术后肺部感染、吻合口瘘、腹腔感染发生率,无显著性差异(P>0.05)。结论 FTS应用于胃癌围手术期患者的营养状况较好,肠道恢复较早、咽痛、恶心、呕吐并发症发生率低,住院时间缩短,治疗费用降低,不增加严重并发症的危险,但切口脂肪液化率偏高。Objective To compare the application of fast track surgery（FTS） and traditional surgery in perioperative period of gastric cancer.Methods Eighty two cases of gastric cancer,who were performed operation from March 2007 to May 2010,were observed.42 cases were treated by FTS philosophy in perioperative period（FTS Group）,and 40 cases were treated by traditional surgery（Tradition Group）.Preoperative evaluation was made in both groups.The results of serum albumin,gastrointestinal coincidence time,length of stay,hospital stay and the postoperative complication were investigated and compared between two groups.Results All the patients have accepted the effect a radical cure of D2＋ standard of gastric cancer.Patients in FTS group had showed higher serum albumin,earlier exhaust time of intestinal tract,lower rate of throat pain,sicchasia,throw up and hospital stay（P0.05）.The rate of postoperative lung infection,anastomotic leak,intra-abdominal infection was similar（P0.05）.FTS guoup had higher rate of incision fat liquefaction（P0.05）.Conclusion FTS applied in perioperative period of gastric cancer could obtain better nutrition condition,intestinal tract convalescence accelerate,lower rate of throat pain,sicchasia and throw up,shorter hospital stay and fewer expenses.At the same time,the dangerous of serious complicating diseases is not increased.Otherwise,incision fat liquefaction is higher.
