鼻内镜下筛泡前径路额窦开放60例临床观察

摘　　要：目的探讨鼻内镜下筛泡前径路额窦开放的临床疗效。方法选择我院2004～2011年间,涉及额窦病例的患者60例,均经过CT等影像学检查证实为慢性额窦炎,且均为初次手术。麻醉成功后,采用鼻内镜下筛泡前径路额窦开放手术。术后对本组患者进行随访。结果本组患者术后均没有出现严重手术并发症。术后1个月随访5,1例患者头痛等症状消失,2例患者头痛等症状较术前明显好转。术后3个月观察额窦口及黏膜情况5,1例患者的额窦口大小均在5.0 mm以上,2例患者额窦口虽<5.0 mm,但是引流通畅,黏膜情况良好。术后随访6个月,通过鼻内镜复查,49例患者额窦口开放良好,额窦口均在5.0 mm以上,患者的头痛、鼻塞和流脓等临床症状均消失。结论鼻内镜下筛泡前径路额窦开放术治疗额窦炎,具有解剖结构容易辨认、手术安全、临床效果好等优点,值得临床应用。Objective To investigate the pathway of endoscopic frontal sinus ethmoid bulla before the clinical efficacy.Methods Our hospital from 2004 to 2011 involving 60 patients with frontal sinus cases,have to undergo a CT and other imaging examination confirmed chronic frontal sinusitis,and are the initial surgery.After the success of anesthesia,we use endoscopic frontal sinus ethmoid bulla pathway before surgery.This group of patients were followed-up after surgery.Results The patients were no serious complications.After 1 month follow-up,51 patients with headache and other symptoms disappeared,headache and other symptoms in 2 patients significantly improved compared with preoperative.Observed after 3 months of the mouth and mucous membranes of the frontal sinus,51 patients were in sinus mouth size 5.0 mm or more,2 patients with frontal mouth,although less than 5.0 mm,but the smooth drainage,mucous membrane in good condition.Patients were followed up for 6 months,by endoscopic review,49 patients with frontal sinus ostium good,frontal mouth were 5.0 mm or more,patients with headache,nasal congestion and pus and other clinical symptoms disappeared.Conclusion Endoscopic frontal sinus ethmoid bulla pathway before treatment of frontal sinusitis with easily identifiable anatomical structures,operation safety,clinical effectiveness,and good,worthy of clinical application.
