常规超声及超声造影诊断肾上腺外伤的应用价值

摘　　要：目的探讨肾上腺损伤的常规超声和超声造影表现及其诊断价值。方法回顾性分析13例疑诊肾上腺损伤病例的常规超声和超声造影表现及随访结果。结果 13例疑诊病例中11例超声造影诊断为血肿,2例诊断为肾上腺肿瘤。动态观察:7例3个月复查血肿消失,4例6个月复查血肿消失,肾上腺形态恢复正常;2例病灶缓慢增大。结论肾上腺外伤性血肿各期超声表现有一定特征,超声造影可对其诊断与鉴别诊断提供可靠依据,有利于指导临床制定治疗方案。Objective To discuss the expression and values of Ultrasonography and contrast-enhanced ultrasound （CEUS） used for adrenal trauma. Methods The xpressions of uhrasonography and CEUS used for 13 suspected patients with adrenal gland trauma and the follow-up results were retrospectively analysed. Results Among 13 suspected cases, 11 cases were diagnosed as hematoma by CEUS, and 2 were judged as Adrenal Tumors. After dynamic observation, disappearance of hematoma was found in 7 cases during the counter-check 3 months later and in another 4 patients during the counter-check 6 months later, while the adrenal shape returned to noymal, and 2 patients showed slow enlargement of focus. Conclusions Adrenal trauma-induced hematoma shows specific features in each stage of ultrasonic findings, CEUS can offer reliable basis for the diagnosis and judgment, which is thus good for clinical treatment plan.
