硬膜外腔药物注射对大鼠肾上腺皮质功能的影响

摘　　要：目的了解硬膜外腔药物注射对SD大鼠肾上腺皮质功能的影响。方法雄性SD大鼠72只,随机分为6组,每组12只。组1:利多卡因、维生素B1、维生素B12、生理盐水;组2:组1加醋酸曲安奈德;组3:组2减维生素B12;组4:组2减维生素B1;组5:组2减维生素B12、维生素B1。组6为空白组。硬膜外腔注药,每周1次,连续注射3周,第4周统一采血,测血清皮质醇浓度,并随机抽取4只大鼠处死,取肾上腺行H-E染色,显微镜下观察。结果血清皮质醇含量方面醋酸曲安奈德组、维生素B12组与其余组相比有显著性差异(P<0.05);第2、3、4、5组大鼠肾上腺皮质束状带都明显变窄,且有较多束状带细胞变性坏死。结论硬膜外腔注射醋酸曲安奈德会引起雄性SD大鼠继发性肾上腺皮质功能减退,临床上维生素B1、B12用于硬膜外腔注射是否安全值得商榷。Objective To study the effects of epidural drug injection on adrenocortical function of Sprague-Dawley（SD）rats.Methods A total of 72 male SD rats were randomly devided into 6 groups,and each group had 12 rats.The rats in the 1st group received lidocaine,vitamin B1（VitB1）,vitamin B12（VitB12）and normal saline（NS）;2nd group received triamcinolone acetonide acetate（TA）and durg in 1st group;3rd group： durg in 2nd group except VitB12;4th group： durg in 2nd group except VitB1;5th group： durg in 2nd group except VitB12 and VitB1;6th group was blank group.The rats in the 1st,2nd,3rd,4th and 5th group received drug by epidural injection every week for three weeks,and the concentration of cortisol were detected after the serum of all rats were collected and 4 rats selected randomly from every group were killed in the fourth week.Observing all the adrenals by microscope after they were removed and HE stained.Results There were significant difference between groups included TA or/and VitB12 and other groups on the concentration of cortisol（P0.05）;The adrenal cortex-zona fasciculate of rats in groups 2,3,4,5 narrowed obviously,and many cell degeneration and necrosis in zona fasciculate.Conclusion The adrenal cortex of male SD rats have secondary hypofunction after epidural TA injection,and if it is safe that epiduralVitB1 or VitB12 injection to human is worthy of discussion.
