改良单开门加肌蒂移植预防硬膜外瘢痕粘连的试验研究

摘　　要：目的观察改良单开门加肌蒂移植方法在预防硬膜外瘢痕粘连中的作用。方法将18只家犬随机分为3组,A组每只家犬上行改良颈/腰椎单开门加肌蒂移植术;B组每只家犬上行改良颈/腰椎单开门术;C组每只家犬上行颈/腰椎板切除术。术后4周、8周、12周取A、B、C组中各2只家犬,获得四组改良颈/腰椎单开门加肌蒂移植模型,四组改良颈/腰椎单开门模型和四组颈/腰椎椎板切除模型,分别进行肉眼瘢痕粘连度评级、光镜检查,观察各组硬膜外瘢痕粘连情况。结果改良单开门加肌蒂移植术后硬膜外无瘢痕生长迹象;改良单开门模型有部分瘢痕生长;椎板切除术后硬膜外有明显瘢痕生长。结论改良单开门加肌蒂移植手术在预防椎板切除术后硬膜外瘢痕粘连中有预防作用。Objective To investigate the effect of the treatment of meliorating opening-single door laminoplasty and muscle pedicle transplantation in the prevention of epidural adhesion postlaminectomy.Mehtods 18 dogs were divided into 3 groups,the treatment of meliorating opening-door laminoplasty and muscle pedicle transplantation on cervical/lumbar spine were performed in dogs from group A.Meliorating opening-single door lamioplasty were performed on cervical/lumbar spine in group B,and the laminectomy of cervical/lumbar vertebral canal was performed in group C.Dogs in A、B、C groups were killed with air embolize method after narcosis at 4th、8th、12th week.Each dog＇s epidural adhesion postlaminectomy were observed grossly and histologically.Results Observed histologically,there was no obvious epidural adhesion in the dogs after performed with meliorating opening-door laminoplasty and muscle pedicle transplantation;A few epidural adhedion in the dogs after performed with meliorating opening-single door lamioplasty;Severe epidural adhesion at the dogs after performed with laminectomy only.Conclusion Meliorating opening-single door laminoplasty and muscle pedicle transplantation has been proved to have effect in preventing the epidural adhesion postlaminectomy.
