摘  要:目的探讨D二聚体(DD)、纤维蛋白原(FIB)及C反应蛋白(CRP)联合检测在糖尿病。肾脏病(DKD)的早期诊断中应用价值。方法测定96例T2DM患者(1、2DM组)及50名健康体检者(对照组)的空腹DD、FIB及CRP水平,并同时收集24h尿液进行尿微量蛋白测定。根据尿白蛋白排泄率(UAER)将T2DM分为3组:单纯糖尿病组(SDM组)、早期糖尿病肾脏病组(EDN组)和临床糖尿病肾脏病组(CDN组)。结果T2DM各组患者DD、FIB、CRP水平较对照组升高(P〈0.05),且随UAER增加而增加,3组各指标差异均有统计学意义(P〈0.05);T2DM各组中,3项联合检测的诊断阳性率明显高于单项检测(P〈O.05)。T2DM各组中UAER与DD、FIB及CRP水平均呈正相关(P〈0.05)。结论DD、FIB及CRP联合检测可明显提高糖尿病肾脏病的诊断阳性率,对DKD的早期诊断、病情发展的分析与监测具有一定的指导意义。Objective To explore the applied values of simultaneous detection of plasma D-dimer (DD), fibrinogen (FIB) and C reactive protein (CRP) levels in early diagnosis of diabetic kidney disease (DKD). Methods The levels of DD, FIB, CRP and 24-h urinary microalbumin were deter- mined in 96 DKD patients and 50 healthy controls. According to the urine albumin excretion rate (UAER), all the patients were divided into three groups including simple diabetic group (SDM group), early diabetic nephropathy group (EDN group) and clinical diabetic nephropathy group (CDN group). Results The levels of plasma DD, FIB and CRP in the three groups were higher than those in healthy controls (P〈0. 05), and increased with the increased UAER. There was significant difference in plasma DD, FIB and CRP levels among SDM, EDN and CDN groups (P%0. 05). The positive rate of simultaneous detection was higher than that of single detection (P d0. 05). The UAER was posi- tively correlated with the levels of DD, FIB, CRP among the three groups (P〈0. 05). Conclusions Simultaneous detection of DD, FIB and CRP levels may improve the positive rate of DKD, and it has certain instructional significance in the early diagnosis, analysis and monitoring of DKD.
